0177165220 2013-06-18 UrbanProduction Wells No Violations The producing unit consists of a wellhead with pump jack and storage tank battery. The wellhead and battery are not enclosed. There is a partially dismantled chain-link enclosure at the battery. The battery is comprised of three above-ground storage tanks (ASTs), 1 140 barrel, 1 100 barrel, and a mechanical separator. The battery is shared with the LC Rod & Gun 5 Unit. The battery is also shared with the Klemenc #1 Unit (20865), but is not identified on the tank battery ID. The tank battery ID is lacking any API #s. I called the pumper (Ron Slother) and advised him about the need for maintenance.

1023619642 2016-04-13 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: one wellhead with pump jack system located in a open agricultural field 1,500 feet north of the associated battery (shared with the #2-1092 and the #2-0865). Fittings and connections at the wellhead were secure, with the following exception: The flexible supply line from the well to the pump jack motor was worn, and leaking gas. The battery is comprised of one 210-barrel production tank, and one 100-barrel brine tank, with separator with associated appurtenances. Load lines were plugged and secure. Secondary containment was intact. Signage was present. Production reports were current. I notified M&R Investments about the leaking gas supply line. I noted that the signage at the wellhead has not been changed to reflect the new owner. I notified the owner about the need to update the signage at the wellhead.
1049920048 2013-02-11 UrbanProduction Wells No Violations The producing unit consists of the following: a wellhead with pumpjack system, a 4 tank battery (2 210 barrel, 2 100 barrel), 3 mechanical separators, and associated piping and mechanicals. The tank battery is shared with the #1092 and #0865 units. The tank battery is partially enclosed in a 6-foot chainlink fence. The tank battery secondary containment is adequate. The unit spacing is in accordance with the rule. I did not observe any signs of leaks, releases, or other problems. The unit ID does not indicate permit numbers. There is no ID at the wellhead. I called Matt Sisson and notified him of the need for ID.
1217164465 2014-03-13 UrbanProduction Wells No Violations The unit is producing. The unit identification placard is not compliant. The placard at the tank battery is missing the API #s. The placard indicates the Lake County Rod & Gun Club #4 and #5 produce into the battery. Inspection indicates the Klemenc (#20865) is also producing into this battery but not identified. I will contact Broadstreet about adressing the issue.
1469543205 2015-03-25 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The wellhead and are located on a larger farmstead parcel with access via an unpaved, dedicated access road from E. Loveland Road. The road is mostly grown-over with field grass but is passable when dry. The wellhead is comprised of a pump jack system. Fittings and connections are secure. The tank battery is comprised of 1 210-barrel steel production tanks, 2 100-barrel steel brine tanks and 3 separator units. The battery is shared with the #2-0865 and the #2-1092 wells. Elements of the system are aged and show some signs of minor to moderate corrosion but are intact. Fittings and connections are secure and free from signs of leakage at the time of this inspection. The load-outs are plugged and secure. ID signage and secondary containment are present. Areas surrounding the wellhead and battery are reasonably well drained and free from signs of adverse impact from the on-going production activity associated with this well.
1511284178 2017-11-21 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on otherwise undeveloped rural property, with the battery located next to the road, and the well beyond the entrance to the right. The battery is shared with the #2-1092 and the #2-0966 wells, located on the same parcel. At the time of this inspection, I found the following: a wellhead pump jack system and associated battery. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and secure. ID signage was present and at the battery but needed updated and complete API number. The secondary containment was intact with no signs of leakage, spills, or problems. The well was in active production. The chart recorder was active. Production reports were current. Bonding and insurance was in place and current. I notified Bill Fitzmartin about the need for updated signage.
