0567276198 2017-07-18 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. The well was plugged and abandoned in 5/2013. At the time of this inspection, the area where the former well had been located has been restored with asphalt paving and the area of the former battery has well-established grass and other vegetation. No further activity is needed. A waiver has been submitted for the sales line, which was left in place. Final Restoration is passed.

1124635908 2012-11-28 UrbanProduction Wells No Violations The unit consists of a pumpjack unit in a 6-foot fenced enclosure. The tank battery (TB) consists of 1 210-bbl tank, 1 and mechanical separator. There is not an enclosure at the tb. The spacing is in accordance with the rule. The tank battery dike is adequate. There is a minor leak at the bottom of the production tank. There is no ID at the TB or the pumping unit. I called Cindy Moore of Allied Oil Field Serv., regarding the need for locks on the load-outs, repair of the leak,and missing ID. She indicated that they are beginning to implement the repairs.

There is a 50-barrel poly tank remaining in the area of the former tank battery. There are former supply lines sticking up through the ground next to the former tank battery.
