0003169181 2017-04-17 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves. At the time of this inspection, I observed the following: The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.

At the time of this inspection, I observed the following: the load line valves were locked and the ID signage had been updated. The items noted in the compliance notice had been addressed. I did note that there was no enclosure at the wellhead or battery and will discuss with the owner the reason for the missing enclosure.

1114604910 2005-02-14 Production Wells No Violations tank battery check
1346167520 2012-08-27 Production Wells No Violations The unit is comprised of a two-tank battery (1, 210-barrel production and 1, 100-barrel brine) and a wellhead with pump jack. The well is currently in production. I observed the wellhead, tank battery, and lease road areas to be in generally good condition. The tank and well spacing is in accordance with 1509:9-9-05. Unit identification is present and legible. The tank battery dike is in good condition. I did not observe any leaks, spills, or other signs of contamination. The load out valves needs locks. I called Jim Williams regarding the locks. He indicated he would fix them next week.
1457542184 2016-03-08 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I noted the following: The well is comprised of a wellhead pump jack and tank battery with one 210-barrel production tank and one 100-barrel brine tank. Connections and fittings are secure and free from signs of leakage or problems. The load-line valves were plugged but not locked. The secondary containment was intact. The owner must take the following action by 04/16/2016: lock and secure all load lines in accordance with OAC 1501:9-9-05(E)(3). Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

1587275662 2015-02-06 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well is located on the south side of Apollo Parkway. in an otherwise undeveloped portion of an area comprised of commercial and light industrial development. The wellhead and battery are located approximately 300 feet to the south, after entry to the lease road. My inspection noted the following: The well is comprised of a wellhead pump jack and tank battery with 1 210-barrel production tank and 1 100-barrel brine tank. At the time of my inspection, the area is covered with up to 14 inches of snow and, as such, elements of the system are obscured with snow cover. Connections and fittings are secure and free from signs of leakage or problems. Evaluation of the condition of the battery interior, lease road condition, and other surficial features is not possible due to snow cover.
