0066121655 2015-05-27 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued Compliance Notice. The well is comprised of a wellhead with mechanical plunger lift system in a small, chain-link enclosure. The interior of the enclosure is overgrown with dense vegetation. The tank battery is comprised of one 210-barrel steel production tank and vertical separator unit in a chain-link enclosure. The interior of this enclosure is also overgrown with dense vegetation., There is no identifying signage anywhere on this unit. According to discussions with Future Productions, changes made by Dominion/EOG have rendered it no longer possible to connect to the local sales distribution system. Further, discussions with the landowner indicate that no gas is being provided for domestic use. There has been no attempts by the well owner to address the issue of non-compliance. Im recommending a Chiefs Order be issued for enforcement.

As part of the Compliance Notice, Future Productions, Inc. (Owner) was directed to plug and abandon in accordance with Ohio Revised Code (ORC) 1509.12B. As part of the Compliance Notice, Future Productions, Inc. (Owner) was directed to plug and abandon in accordance with Ohio Revised Code (ORC) 1509.12B. The owner was given a compliance date of 6 months from the date of the issuance of the Compliance Notice. At the time of this inspection, I, along with Northeast Regional Supervisor Tom Hill, met with property owner Robert Slowey, at the location. Mr. Slowey indicated that Future Productions has not maintained the well, and had not been responsive to his attempts to contact them. He further indicated that he wished to have the well plugged and abandoned. I observed the well to be idle and in a state of disrepair, and the stipulation of the Compliance Notice to plug and abandon the well had not been complied with. I contacted Brett Huntsman on 05/01/2018 who had not responded to a phone message, at the time of this report filing.

At the time of this inspection, I noted the following: the well and associated production elements consisting of a 210-barrel steel stock tank, mechanical separator, and concrete brine vault remained in a similar condition of disuse and disrepair. There has been no apparent attempts by the owner to obtain a permit to plug the well. There remains no active insurance.

We agreed the Owner must establish a course of action within two weeks, or by 06/16/2017.

At the time of this inspection, I observed the following: one wellhead with plunger lift system located in a small chain-link enclosure. The interior of the enclosure was overgrown with vegetation. The flow-line connection was closed at the wellhead. There is a domestic supply connection at the wellhead which was open. The battery was comprised of one 210-barrel steel tank, and a separator. Both the interior of the battery and surrounding area is overgrown extensively with vegetation. No production has been reported since 2012. There was a domestic supply connection at the house located at 3950 River Road. The domestic supply was closed at the house. The well owner has not responded with a plan to return the well to production.
1156409502 2014-11-05 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The wellhead plunger lift and battery are located on within a farmstead. The wellhead is located in a small chain-link enclosure. The interior of the enclosure is overgrown with vegetation obscuring much of the well from visual inspection. The battery is comprised of 1 210-barrel steel tank, and a separator. Both the interior of the battery and surrounding area is overgrown extensively with vegetation. No production was reported for 2013. The landowner has reported that there is no receipt of domestic gas from the well. Future Productions has reported that the infracture to sell commercial gas has been cut off. As such, the well owner must plug and abandon the well with 180 days or be subject to additional enforcement action.
1309712424 2016-12-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice. At the time of the issuance of the compliance notice, it was noted that there had been no reported production since 2012 and subsequent discussions with the well owner indicated that sales line connection had been truncated and could not be re-connected, rendering commercial production not possible. Further, the landowner has not been receiving domestic gas. Subsequent discussions with the well owner have indicated that the well has become uneconomical to commercially produce. At the time of this inspection, the fittings and connections at the wellhead were closed. The interior of the wellhead enclosure was overgrown with vegetation. Conditions at the tank battery had not changed. Elements consist of one 210-barrel steel production tank, an in-ground, cement brine tank, and separator unit in chain-link enclosure. The interior of the tank battery was overgrown with vegetation. There was no readable identifying signage. There was no evidence that on-going maintenance had been occurring.

At the time of this inspection, the well was not in production and all valves on the well and flow lines were shut in. Tank battery location had a 210 BBL tank, concrete brine vault and mechanical separator. Valves at the separator were also closed. There is a current open compliance notice to plug this well dated 11/14/2014. The required plugging for this well was 5/15/2015. At the time of this inspection, the well was not in production and has not been plugged.
