0398100994 | 2017-04-25 | UrbanProduction Wells | No Violations | I inspected this well location as a follow-up to a previously-issued Compliance Notice. At the time of the compliance notice, I had noted the following: the production tank was severely corroded with several perforations. In addition, the interior of the tank battery had become overgrown with vegetation, and the unit ID signage had not been updated to reflect the change in ownership. At the time of this inspection, the corroded tank was removed, the signage was updated, and the battery vegetation had been removed. John Raber with M & R Investments has indicated that efforts are on-going to return the well to production. |
0642308498 | 2015-02-04 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. The wellhead and battery are located on the east side of St. Rt. 528. The battery is adjacent to the roadway and the wellhead is located in a farm field and accessed via the same dedicated lease road. At the time of this inspection, the surrounding areas are covered in 8 to 12 inches of snow which obscures some portions of the unit. I observed the valves and connections at the wellhead and the battery to be secure and free from signs of leakage or other problems. |
0762596026 | 2016-09-29 | UrbanProduction Wells | Violations Noted | I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system and tank battery in separate chain-link enclosures. The well was shut-in and had not had in production since 2009. The tank battery was comprised on one 210-barrel steel production tank, one 140-barrel steel brine tank, horizontal separator, and associated production system appurtenances. The production tank has become severely corroded and has developed several perforating holes, which have compromised the integrity of the tank. In addition, I observed that the interior of the well and battery enclosures was overgrown with vegetation. The ID signage reflects the previous owner, and has not been updated. The owner must do the following by 10/31/2016 or be subject to additional enforcement action: replace the 210-barrel production tank, update the signage to reflect current ownership and contact information, remove the overgrowth of weeds within the individual enclosures. Contact Inspector Stephen Tompkins at 330-907-4647 with any questions about this inspection. |
1239929044 | 2018-08-21 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. At the time of this inspection, I noted the following: Fittings and connections at the wellhead and battery were secure and free from signs of leakage. The well was not in active production. I noted that the well ID signage had become faded and unreadable. I also noted that the well interior had become overgrown with vegetation. I advised Bill Fitzmartin with M&R Investments about the need for maintenance. He indicated they would remedy as soon as possible. |

1317851759 | 2011-05-03 | UrbanProduction Wells | No Violations | Tank battery in good repair. Well is not in production. Valve on wellhead is leaking. Contacted Matt Sisson on 5/6/11 to address the issue. Revisited site, valve replaced. |
1347033652 | 2012-09-05 | UrbanProduction Wells | No Violations | I observed a tank battery (TB) consisting of one, 210-barrel (BBL) production tank, one, 100-BBL brine tank, and a mechanical separator unit. The unit is shut-in. There is an identification sign at the TB. The dike appears to be adequate but is overgrown with weeds. The gate is locked. I did not observe any signs of leakage or venting gases. The wellhead is located 100 feet north of the TB. The wellhead consists of a typical valve stem assembly with a plunger lift unit. There are no signs or identification at the wellhead. I did not observe any evidence of gas or oil leakage at the wellhead. The wellhead is enclosed within a 6-foot, chain link style fence. I called Luke about maintenance and he indicated that the well is likely to be plugged. |
1347034807 | 2012-09-05 | UrbanProduction Wells | Violations Noted | I observed a tank battery (TB) conisting of one 110 barrel (BBL) production tank and a mechanical seperator unit. The unit appears to be shut-in. There are no signs or indentification. I did not observe any evidence of gas or oil leakage. The unit appears to be in disrepair. The area within the dike is overgrown with vegetation which has likley compromised the integrity of the containment. I was not able to locate the production wellhead. |
1377701864 | 2013-08-28 | UrbanProduction Wells | No Violations | The interior of the tank battery is overgrown with weeds. The unit ID placard is peeling and not readable. |
1395068670 | 2014-03-17 | UrbanProduction Wells | No Violations | The unit is producing with no indications of leaks or spills. The weed overgrowth has been tended to. The ID placard is still not readable. I notified Ed Butler of the need for a compliant ID and he said he would add it to the maintenance list. |
