At the time of this inspection, I noted the following: The load lines remained unlocked and unsecure. The ID signage continued to reflect the prior owner of the well. The new owner has made pledges to correct the deficiencies, but with no results at the time of this inspection. The well was shut-in and not in active production.

During this inspection, it was observed that the well was equipped with a plunger lift assembly and not in production. The tank battery was comprised of one 210 BBL tank, one 100 BBL tank and a mechanical separator. The load lines from the tanks have not had locks added to them. The diked tank battery was fenced but still needed the identification to be updated with the current company information. The tank battery was shared with well 2-0226.

At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system in a board-on-board enclosure. A tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, a mechanical separator. The battery is partially enclosed in a chain-link fence. Fittings and connections were secure. Load lines were bull-plugged. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. The battery is shared with the #2-0226 well which is located in the front of the industrial facility. The ID signage needs to be corrected to reflect the correct well name. Ive notified the well owner about the name correction.

The well is located along a light industrial/commercial corridor on Tyler Blvd. in Mentor. The wellhead and battery is located behind the facility at the referenced address. The wellhead plunger lift and battery are located in a common chain-link enclosure. The battery is comprised of a 210-barrel production tank and 100-barrel brine tank, and separator unit. Load-outs are plugged and secure. The battery is shared with the #2-0226 well which is located near the front portion of the facility. The ID signage identifies this well as the Worthington, which conflicts with other records. I will follow up with permits to resolve. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. I did not observe any evidence of impact relating to on-going production activity.
2138046739 2017-12-06 UrbanProduction Wells Violations Noted I inspected the well location as part of routine inspection activity. The well and associated battery are located behind a commercial building, in a fringe area at the back of the parking lot. The battery is shared with the #2-226 well, which is located near the front of the building. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery. Fittings and connections at the well and battery were secure and free from signs of leakage. The secondary containment was intact. I noted the following deficiencies: the load-lines were not locked as required. The ID signage, did not have correct information, the well name was incorrect, a had not been updated to reflect the change of ownership that occurred over 12 month prior to this inspection. The owner must place locks at the load-line valves and place compliant unit identification at the well within 30 days or be subject to additional enforcement.
