0542958531 2018-01-30 Production Wells No Violations Requested a permit number be assigned through the Columbus Office. I met DOGRM County Inspector Mark McCombs and we ran a magnet down the hole to an approximate depth of 25 feet. We could detect no metal. The tenant farmer brought out boards to cover the top of the hole. Request has been made thru the New Philly Office, DOGRM program administrator Gene Chini to obtain emergency contractors for a 10 foot x 10 foot x 6 foot height security fence with warning signage. I took one photo.
0821892381 2018-03-02 Orphan No Violations The area around the hole opening is becoming more unstable, soft, and is still caving from above and below.

1517948588 2018-01-29 Production Wells No Violations Sink hole found by tenant farmer and turned in to the Division through the Knox County USDA soil service. Site inspection with DOGRM Inspector Eric Daugstrup. The surface hole has a 3 foot diameter and a weighted line on a counter reel was run to 25 feet. The hole appears to taper down to a third or less of the top diameter. From the surface there is no casing, gas odors, or fluids visible. I made a request through the Columbus Office for a OFG map for Morgan Township of Knox County. This hole is close to a well spot on that map. This hole is unstable and an imminent danger to public health and safety. I took 5 photos.

1518469807 2018-02-12 Orphan No Violations I met with Inspector Jeff Fry to check the status and depth of this well bore. The hole had caved in and filled in more and we could not get a line down to try and tag any pipe with a magnet. The opening has widened as well. We could only get down 25 feet.

1865677034 2018-06-13 Plug / Plug Back No Violations The plugging operations continue with this orphan well. The excavation work continues with the plugging of this well.
2034943162 2018-02-02 Production Wells No Violations Met DOGRM Emergency Operations Coordinator Jason Blocker to run the Divisions down hole camera. The camera was ran to a depth of approximately 30.5 feet. The video will be compiled and studied for evidence of an oil and gas well. This appears to be a Historic Production well.
