1476199932 2016-10-11 Casing String Witnessed No Violations On the day of my inspection I found that Dugan Drilling had 8.625 inch casing set at a depth of 488 feet inside of 10 inch hole. Cemenco pumped a mixture of gel and cotton seed hulls prior to mixing cement. The well did not circulate during the mixing stage. They mixed 80 sacks of class A cement and pumped it down the well. The well did not circulate until the displacement stage. There were two brief moments where gel and cotton seed hulls were circulated to surface during displacement, but the mixture fell back down the annulus as soon as Cemenco stopped pumping. I spoke with Knox County inspector Mark McCombs. He claimed that the area has a history of the cement falling to 40 to 60 feet. I told Dugan to let the cement set overnight, and top it off with sack cement. They agreed to top it off the following day after the casing pressure test.
1476285903 2016-10-12 Drill / Deepen / Reopen No Violations On the day of my inspection I met Guy Dugan at the site to witness the pressure test and the casing being topped off with sack cement. Cemenco pumped into the casing and brought the pressure up to 300 psi. The pressure bled off to 150 psi. within 5 minutes. Dugan Drilling tightened the cement head down on the casing and Cemenco tried to bleed all of the air out of the system. They pressured up on the casing again and the test failed again. I spoke with Guy Dugan and requested that he have a bond log run on the surface casing cement job.
1476452675 2016-10-14 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Dugan Drilling was not working at the drill site. The cement head had been removed and the 8.625 inch casing was open. The fluid had fallen out of sight. I dropped a small rock and estimated the fluid to be down 150 feet. I also dropped a pebble down the annulus and estimated the cement to be down 100 feet. I called Guy Dugan and he said they were still waiting on Jet Wireline to run the bond log.
