0093281363 2018-08-15 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 284 has moved off of location. Contractors were onsite pressure washing the rig matting for transport.
0135109741 2018-08-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 284 tripping drill pipe out of the hole. They drilled to a total depth of 18160 feet. This is the last well on pad. I did not make identification to Company man Mark Smith because they were having a rig move meeting. Walk around location revealed no issues. Production casing cement job will be later this week. Mud weight was 15.2 ppg. No flare burning during this inspection.
0196254628 2018-07-31 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 284 on location. Their mud pumps are down and not drilling. They have drilled 106 feet of new hole in the last two days. Current depth is 9080 feet. Heavy rain during this inspection. Pads drains are closed.
0262734413 2018-10-23 Completion Testing No Violations On the day of my inspection I found this pad to contain two wells that are not producing. Both wells have been stimulated but neither have been drilled out. The wells have two frac valves on them. IROC is onsite building production equipment. Surface annular pressure is 0 and intermediate annular pressure is 1000.

Snubbing unit is moving off of location during this inspection. Identified myself to company man Josh McTheny. Surface annular pressure is 0 and intermediate annular pressure is 500.

1502496913 2018-08-29 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain two unlabeled wells with night caps. Pressure gauges are in cellar underwater and unable to be read. Pad appears in good shape and is stable. The outslope of pad opposite the lease road entrance has a patch of cat tails growing, indicating a constant presence of water. ODNR engineering team is aware of the area. Wells have not been stimulated. Pad outslopes are vegetated and a locked fence surrounds location. Site meets requirements of OAC 1509.072A for preliminary restoration.
1524556727 2018-04-24 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1570 feet. Previous casing set and cemented was 18.625 inch diameter mine string casing to a depth of 216 feet. Wyoming casing ran 1563 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot, with 3 foot stick up. Casing was centralized with 13 centralizers. Shoe track is 45 feet long. CJ/Otex was onsite with 1260 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 231 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 38 barrels of cement returned to surface, and floats held with 1 barrel back.
1532436502 2018-07-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 284 drilling a 12.25 inch diameter hole at 3580 feet. They are drilling on air and discussion will Company man Will Brousard revealed they have not encountered any water in this section as of yet. Ground hog hole located during last inspection has been filled back in. Will expects the intermediate job to be up this weekend.
1721327820 2018-10-04 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. I identified myself to company man Matt Bollinger. Wireline is running in to stage 13/47 to set plug and perforate. Plug will be set at 16108, bottom perforation at 16097, and top perforation at 15956. Intermediate annular pressure is 664. Stage 12 was stimulated with 4877 barrels of fresh water and 1000 gallons of 7.5% HCl. 230100 pounds of sand pumped, 59690 lbs 100 mesh and 170410 lbs 40/70 mesh. Breakdown pressure was 9849 at 25 barrels per minute. Average rate was 75 with a maximum rate of 86 barrels per minute. Initial shut in pressure was 7228 and 5 minute ISIP was 6741.
2139647096 2018-04-11 Casing String Witnessed No Violations Pad is at the end of TOWNSHIP ROAD 106, Rayland, Ohio (just east of Mt. Pleasant). On the day of my inspection I witnessed Express had augured a 36 inch diameter hole to a depth of 100 feet. 100 feet of 30 inch diameter conductor casing was run to 94 feet, leaving 6 feet above ground level. Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50% percent excess ran. Cement was displaced with 74 barrels of fresh water. 10 barrels of cement returned to surface.
