2018-10-24 H2O 0 5062 Bbl 230960 Frac Tech Services On the day of my inspection I found FTS onsite tied in to all 3 wells on the pad. I identified myself to company men Bill Pugh and Brady Allen. Stages 1-10 have been stimulated on this well. Stage 10 was completed on 10/24. Plug was set at 21464, with bottom perforation at 21453 and top perforation at 21312. 3000 gallons of 7.5% HCl pumped along with 5062 barrels of slurry. Hole is currently drilled to 9970 feet. Walk around location revealed no issues. Containment functioning as designed. No flare during my inspection. Mud weight is 13 ppg.

They have not spudded into this well yet to start the intermediate section. Recon on location pressure washing mats and rig. Containment is functioning and pad drians are closed.

0503206063 2018-01-23 Drill / Deepen / Reopen No Violations On the day of my inspection I found pad having two auger-type conductor rigs drilling. Company representative Stuart Schwarts said that this well had a 36 inch diameter hole drilled to 68 feet. It was then grouted to surface. It was again drilled to 30 feet and had wet grout in it. Hole was grouted to surface again. After 48 hours it was drilled with 42 inch diameter bit to a depth of 60 feet. Fiberglass tubing was ran and well was cemented back to surface. This date, the well is being augered with a 36 inch bit.

Identified myself to company man Jeff Whitehair. They finished stimulating stage 72/72 on 11/21. Kill plugs were set at 8210 and 8360.Surface annular pressure was 0 and intermediate annular pressure was 250.
1060700935 2018-06-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 was drilling an 8.75 inch diameter hole at a depth of 18375 feet. The rig is drilling with a conventional 2 degree motor/BHA instead of a rotary steerable motor. Kick off point was at 9180 and the curve was landed at 10320. Current mud weight is 14.7 and Company Man Matt Miller believes they will be above 15 ppg when they run casing. They hope to reach total depth of 23033 on Wednesday. No flare is currently burning and they are averaging around 4000 units of gas. Containment is full of rain water and company man said that 2 vac trucks are en route.
1152898552 2019-01-14 Completion Testing No Violations On the day of my inspection I witnessed Hybrid R252 on location. They are drilling frac plugs on this well and just finsihed drilling out #55 of 73. Surface and intermediate annular gauges are shut and read 0.
1519135491 2018-02-20 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1670 feet. Previous casing set and cemented was 18.625 inch diameter mine string casing to a depth of 329 feet. Wyoming Casing ran 1662.7 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot with a 2.7 foot stick up. Casing was centralized with 14 centralizers. Shoe track is 43 feet long. CJ/O-tex was onsite with 1315 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 40% excess ran in open hole. Cement displaced with 246 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 45 barrels of cement returned to surface, and floats held with 1 barrels back.
1583844771 2018-08-29 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 unlabeled wellheads. These wells have not been stimulated. Weld inspector is onsite x-raying the pipeline to the pad. Surface and intermediate gauges are in cellar, underwater, and unable to be read. 5 frac tanks are on location and no other equipment. Pad is stable and vegetated. Site meets requirements of OAC 1509.072A for preliminary restoration.

Plug set at 16418, with bottom perforation at 16407 and top perforation at 16266. Stage design calls for 230000 lbs of sand, 40k 100 mesh and 190k 30/70 mesh. Estimated slurry volume is 5500 barrels with 3000 gallons of 7.5% Hcl Surface annular pressure is 0 and intermediate annular pressure is 350.

Stages 1-10 have been stimulated on this well. Stage 10 was completed on 10/24. Plug was set at 21464, with bottom perforation at 21453 and top perforation at 21312. 3000 gallons of 7.5% HCl pumped along with 5062 barrels of slurry. They pumped 190,980 lbs of 30/70 mesh sand and 39980 lbs of 100 mesh sand. Breakdown pressure was 7987 at 15 barrels per minute (BPM). Max rate was 80 bpm. Average rate was 78 bpm. Max pressure was 11660. Average pressure was 10996. Initial shut in pressure (ISIP) was 6711 and 5 minute ISIP was 6386. I did not witness this stage. Surface annular pressure was 0 and intermediate annular pressure was 430.
1841183962 2018-01-25 Casing String Witnessed No Violations On the day of my inspection I was accompanied by Engineer Technician Jerry Smith. We witnessed Express had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18 50% percent excess ran. Cement was displaced with 75 barrels of fresh water. 15 barrels of cement returned to surface. Had to wait on water trucks for each conductor witnessed.
