Stage 42 frac plug is at 16163, bottom perforation at 16152, and top perforation at 16011. Intermediate annualr pressure is 680 psi. Surface gauge not viewed as there was pressure on the lines for frac.

No flare during my inspection. Mud weight is 13.6 ppg and company man anticipates it will be above 14 ppg before they reach total depth. Walk around location revealed no spills. Containment is functioning and pad drains are closed.

Wireline is in this well currently and will set stage 19 frac plug at 20188 feet, bottom perforation at 20176 feet, and top perforation at 20037 feet. They are running 5 guns with 4 shots on 36 foot spacing. Surface annular pressure is 0 and intermediate annular pressure is 500.

Mr. Ruckert believes they will likely start stimulation operations on Saturday. There is an above ground storage tank onsite. Not all equipment is onsite this date. Surface annular pressure is 0 and intermediate annular pressure is 950. Company an said he would have the pressure bleed off when they get all rigged up.

0772943949 2018-02-23 Preliminary Restoration No Violations On the day of my inspection I found this pad containg four labeled wells. Wells have surface casing set in them and are awaiting a larger rig to finish. The pad is fenced, gated, and locked. All disturbed area is heavily mulched and doing well with all the rain the area has received over the past 2 weeks. Leas road also in good shape. Pad drains are shut. Site meets requirements of ORC 1509.072A for preliminary restoration.

Company man Heath was notified of oil off containment that I located. He stated that it was from a cement truck from the production casing job that blew a hose and they thought it was all cleaned up in the night. He stated he would make sure it was completely cleaned up.

Walk around of location revealed no issues with containment.
1004007775 2018-08-28 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. Identified myslef to company men Wendall Brady and Chris Telford. Stage 40 has been stimulated. There are 77 total stages. Plug was set at 16513, bottom perforations at 16502, and top perforations at 16361. Surface annular pressure is 0 and intermediate annular pressure is 700. 1000 gallons of 7.5% HCL pumped. Breakdown pressure was 7940 at 15 bpm. ISIP was 7129. Average pressure was 10408 with max pressure of 11715. Average rate was 70 bpm. Total clean volume pumped was 3091 barrels. 165630 lbs of sand pumped and it screened off. I did not witness this stage.
1127164516 2018-10-22 Completion Testing No Violations On the day of my inspection I found Deep Well snub #11 onsite. I identified myself to company man James Edwards. This well has had frac plugs drilled out and is awaiting flowback. Surface annular pressure is 400 and intermediate is 550. Production casing pressure was 3300 after drill out. James anticipates moving off of this pad late in the week.

Frack stacks are on wells, an aboveground storage tank, and two frac tanks are onsite. Universal will be coming to this location after finishing the Elite pad. Surface annular pressure is 0 and the intermediate annular pressure is 500.

1491007610 2018-01-12 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1448 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1441 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot and a 3 foot stickup. Casing was centralized with 12 centralizers. Shoe track is 43 feet long. Halliburton was onsite with 1466 sacks of class H Neocem cement. Cement weighed with mudscale at 15.8 ppg and yield of 1.241. 50% excess ran in open hole. Cement displaced with 213 barrels of fresh water. 10 barrels of water pumped ahead of cement. 80 barrels of cement returned to surface, and floats held with 1 barrel back to truck.

Identified myself to company man Gonzo.No changes have occurred on this well since my last inspection on 8/28. Stage 40 was shut down early due to pressurring up. This well is on stage 59/77. Stage 59 was just perforated and wireline is coming out of the well. Plug set at 13188, bottom perforation at 13176, and top perforation at 13036. Surface annular pressure is 0 and intermediate is 600.
