0490700936 2018-07-18 Completion Testing No Violations On the day of my inspection I found Deep Well Snub 11 rig moving onto location. Pad has been fracked and rig will drill out plugs. Surface annular pressure is 0 and intermediate annular pressure is 800.

1305992295 2018-07-12 Completion Testing No Violations On the day of my inspection I witnessed Halliburton onsite. Stimulation is complete on this well and kill plugs were set at 9550 and 9400 feet. Surface annular pressure is 0 and intermediate annular pressure is 596.
1416179492 2018-10-26 Preliminary Restoration No Violations On the day of my inspection I witnessed this contain contains five properly labeled producting wells. Wells do not have tubing in them and are produced on natural reservoir pressure through the 5.5 inch diameter casing. All wells on the pad are producing from each side of the wellhead. Production equipment includes 10 GPUs, 10 sand seperators, 2 500 barrel steel tanks in their own containment, and 1 combustor. Surface annular pressure is 366 and intermediate annular pressure is 557. Site meets requirements of OAC 1509.072A for preliminary restoration.
1509711439 2017-11-02 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 111 feet and ran 111 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex Pumping LLC pumped 390 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack cello-flake. O-Tex then displaced with 80 barrels of water, leaving 15 feet of cement in the casing. Good cement returned to surface and remained static. Approximately 20 barrels of cement returned to surface, Ascent left approximately 18 inches of cement in the cellar as a seal.
1511266280 2017-11-21 Casing String Witnessed No Violations I am on site to witness the cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of approximately 1368 feet and set 1362 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of gel ahead, then pumped 1450 sacks of class A cement, 1.2 yield and 15.6 pound per gallon, 1.5 percent calcium chloride, 0.25 pound cello flake per sack, 5.23 gallons per sack water requirement. O-Tex displaced with 197 barrels of fresh water. Approximately 40 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 988 psi for 5 minutes.
