0082485121 2018-04-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services has 2 auger rigs on location. Both rigs are drilling. The cellar is set and 36 inch diameter hole has been augered to 80 feet. Driller believes they will reach total depth of 100 feet this afternoon and has casing on the way. Company man Doug Bennett plans to set up cement for tomorrow or Saturday.
0183871874 2018-09-25 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Identified myself to Company men Chuck Davies and Delbert Brown. Stage 56/63 is the next stage to be perforated. Wireline will set plug at 10726. Bottom perforation will be at 10714 and top perforation at 10574. Surface annular pressure was 100 and intermediate pressure was 100.
0195408976 2018-05-04 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augered a 42 inch diameter hole to 100 feet and ran 90 feet of 30 inch diameter casing. Express was onsite with 630 sacks of class A cement weighing 15.6 ppg and a yield of 1.18. Cement displaced with 66 barrels of fresh water. No returns to surface. Cement tagged at 64 feet. Dumped an additional 40 yards of 9 sack grout while I was onsite. Cement tagged again at 40 feet. Cement supervisor Brian and company man Doug Bennett onsite. A total of 92 yards was dumped to get cement to surface, ending Saturday morning.
0551163461 2018-08-14 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 had reached total depth of this well at 20437 feet.. Wyoming casing has ran the 5.5 inch diameter production casing and CJ/O-tex is on location to cement the casing back to surface. Final mud weight was 14.9 ppg. Viewed the outslope of pad where Ascent had remediated a seep. Majority of the area is grassed with new vegetation. There is maybe 10 square feet that is not growing. This area will be watched for indication of a continuing issue. Pad has receieved rain yesterday and this morning. Ground was damp.

1229705738 2018-11-05 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing 3 producing wells. Identified myself to company man Shaun Gallager and lease operator Shaun Garrettson. This well is producing off of natural reservoir pressure through the 5.5 inch diameter casing. Surface annular pressure is 181 and intermediate annular is 136. Production casing pressure is 5188. Production equipment includes 3 line heaters, 3 GPUs, 6 sand seperators, and 2 500 barrel steel produced water tanks in their own containment. Wells are producing from both sides of wellhead. Site meets requirements of OAC 1509.072A for preliminary restoration.
1404694829 2018-07-23 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by Paul Logan, Jerry Smith, and Scott Scheiner. Follow up to complaint 5900. The seep on the northwest corner of the pad has been seeded and mulched. Water tested pH of 8 and 99 chlorides. Area received heavy rains last night and water may have had rain water influence. No sample was taken. Pad drain has water accumulated in it with chlorides over 4000. Area will be watched to see if crystalization reforms. Well connect will be coming through this corner and Engineering will have Ascent repair underdrain at that time.
1450523349 2018-09-20 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Identified myself to Company Man Chuck Davies. This well is on stage 43/63. Stage 43 plug was set at 13001, bottom perforation at 12989, and top perforation at 12849. Surface annular pressure was 100 and intermediate was 158.
1467907119 2018-09-12 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Identified myself to company man Gilbert Agular and John Talbot. Wireline is running in well to perforate stage 25/63 from 15999-16151 with plug set at 16151. Surface annular pressure is 100 and intermediate is 450. 5 guns with 6 shots on 35 foot spacing used to perforate.
1525179539 2018-04-30 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed a 36 hole drilled to 100 feet. Hole was too crooked to get casing down. Express dumped 10 yards of 9 sack grout into the open hole. Additional grout will be placed later, and they will try to drill again.
1526303654 2018-05-14 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1412 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 90 feet. Wyoming Casing ran 1405 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot, with a 3 foot stick up. Casing was centralized with 11 centralizers. Shoe track is 45 feet long. C&J/O-tex was onsite with 1420 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 207 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 65 barrels of cement returned to surface, and floats held with 1 barrel back.
1532542300 2018-07-18 Drill / Deepen / Reopen No Violations On the day of my inspection I received a call from land renter Bart Morrison. Entered as complaint number 5900. Mr. Morrison sent me photos of the pad corner with a white crystalization formed. During my inspection I witnessed that a skid steer had cleaned up this material and regraded the area. pH tested 12 on strips. No chlorides appeared on low quantab. Called will be placed to Scott Scheiner -ODNR hydrologist and Paul Logan- ODNR engineer for investigation.
1562901646 2018-08-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 has moved off of location. A crew is onsite pressure washing the mats and stacking them for removal. Pad drains are shut. Containment is still in place. Seep on outslop of pad mentioned in previous ispection was viewed today. Approximately 40 sq ft of reseeded area is still not growing and the soil is moist. Made a small depression with my boot to test pH. pH was 12. Pictures were sent to Hydrologist Scott Scheiner and Engineer Paul Logan.
1619611052 2019-06-07 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain three producing wells. Well is producing through production casing and is flowing from both sides of the wellhead. Well is producing off of natural reservoir pressure. Production equipment includes 3 line heaters, 3 GPUs, 6 sand seperators, 2 500 barrel steel tanks in their own containment, and 1 combustor. The 13.375x9.625 annular pressure is 175 psi and the 9.625x5.5 annular pressure is 7 psi. Site is stable and vegetated. Pad meets requirements of 1509.072(A) for preliminary restoration.
1783439184 2018-07-31 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 drilling a 12.25 inch diameter hole at a depth of 6873 feet. The seep area at the base of pad slope mentioned in previous reports is growing grass. Current pipeline activity likely to redisturb the area. Pad drains are shut and accumulating water aprroixmately 1 foot above the underain outlets in the basin. Company Man Ralph was asked to have them sucked down. This level was before it started raining.

1948180038 2018-10-02 Completion Testing No Violations On the day of my inspection I witnessed the Halliburton frac crew has moved off of location. Three wells are on location. Surface annular pressure is 100 and intermediate annular is 0. Completions rig is expected on location late today. Returned to this pad later in the day to collect a sample (DEP201) of water seeping from the outslope of the pad. Amanda Finn and Greg McCutheon, both of Ascent, were on location briefly while I was filtering the water sample. Sample was delivered to New Philadelphia office and transferred to Kyle Tharp. Ascent has submitted a plan to fix this seep and DOGRM Engineers have approved their plan. Construction is slated to start 10/5.
2051995122 2018-08-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 drilling an 8.5 inch diameter hole at 13352 feet. ID to company man Micheal Rollins. Rig has been having issues with their bit and just made a trip. Mud weight is 14.9 ppg. While on location I viewed the outslope of pad where material was removed and reseeded. Area is growing with vegetation. Ground was soft but it rained yesterday and during this inspection. I will continue to watch the area.
