0312898444 2018-06-12 Preliminary Restoration No Violations On the day of my inspection I witnessed that the pad has been brought online. Pad has 5 properly labeled wellheads. Production equipment includes two 500 barrel steel tanks, five GPUs, two line heaters (for the 5H and 7H wells), and seven sand seperators. These wells do not have tubing in them and are produced through the 5.5 inch diameter production casing. Set up appears to be a frac stack with well head on top of it. Flowback operations onsite have a target goal of 19.1 mmcf/day for this well. Surface annular pressure is 253 psi, intermediate annular pressure is 853, and production pressure is 5609 psi. Flowback is finished on this well. Pad is stable and vegetated. Site meets requirements of 1509.072A for preliminary restoration.

1458966343 2018-03-13 Preliminary Restoration No Violations On the day of my inspection I found a Universal frac crew setting up the pad for frac job. Water tanks and containment are onsite, no frac pump. 7-15 frac stacks are on all wells. 2H, 5H, and 7H are the first wells planned for stimulation. Outslopes of pad are vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
1497351664 2017-06-12 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 330 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 75 barrels of water, leaving 10 feet of cement in the casing. Good cement returned to surface and remained static. Cement was brought to approximately 7 feet below ground surface, leaving a approximately 18 inches of good cement as a floor in the cellar.
1504435450 2017-09-03 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Falcon Drilling rig 26 drilled a 24 inch diameter hole to a depth of 185 feet and set 168 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 30 barrels of gel then 425 sacks of class A cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. Two voids were identified from 103 feet to 106 feet and from 130 feet to 135 feet, two baskets were set at 108 feet and 106 feet to avoid the upper void (conductor to 100 feet). O-Tex displaced with 49 barrels of fresh water to land the plug. Cement did not circulate to surface. Falcon ran approximately 100 feet of one inch tremie pipe down the backside of the 18.625 inch casing. O-Tex pumped 5.5 barrels of class A, 1.2 yield, 15.6 pound per gallon cement to set the basket. After 3 failed top out attempts I left the job, they will continue to top out until they get returns to surface. On the fifth top out cement returned to surface. Total top out cement used was 400 sacks.
1504637133 2017-09-05 Casing String Witnessed No Violations I am on site to witness the cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of approximately 1400 feet and set 946 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of gel ahead, then pumped 1000 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 1.5 percent calcium chloride, 0.25 pound cello flake per sack, 5.23 gallons per sack water requirement. O-Tex displaced with 138 barrels of fresh water. Approximately 11 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1047 psi for 5 minutes.
1789794252 2018-01-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 284 drilling an 8.5 inch diameter hole at a depth of 9910. Location had some oil base mud and diesel on containment near the shale shakers and Company man Van Dietrich was asked to have it cleaned up.
