0819867816 2017-09-21 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigged up on all 3 wells on the pad for hydraulic fracturing operations. While I was on site I witness BJ Services pump the frac job for stage 11 of 54 on this well and stage 11 of 56 on permit 3408120723. BJ Services pumped approximately 351000 pounds of sand with approximately 6357 barrels of water at rates as high as 85.5 barrels a minute during this stage. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0823064549 2017-05-07 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1515 feet and set 1506 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1600 sacks of class H cement, 1.19 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 85 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1150 psi for 5 minutes.
0908823606 2017-07-17 Drill / Deepen / Reopen No Violations This is a follow up inspection to determine if the containment issues found on 7/13/17 have been repaired. On the date of my inspection I found Patterson drilling rig 292 tripping out of the hole to run the cement bond log for the 13.375 inch casing string. They reached total depth for the intermediate string (9056 feet) on 7/16/17. The flooded areas noted in the inspection from 7/13/17 have been drained and the containment on the northwest corner has been extended up the pad berm to fully contain any more high water that might accumulate.
0970521998 2017-07-13 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 drilling the intermediate string of this well at approximately 5720 feet. The Ascent company man indicated that they should reach their total depth on 7/14/17. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. All of the chemicals are being stored within containment. However, the northwestern side of the pad is flooded. This is where the chemical storage is and the far northern corner of the flooded containment is not tall enough to stop the water from draining into the French drain. I notified the company man who said he would have a crew called in to repair the plastic containment as soon as possible. Recent heavy rains are making it hard to drain the area enough to extend the containment to a sufficient height. The pad drain valves are in the closed position and do not appear to be leaking.

1649268076 2017-04-30 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 400 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 15 barrels of water, leaving 10 feet of cement in the casing. I witnessed good cement return to surface and remain static.
