0038673901 2017-09-21 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigged up on all 3 wells on the pad for hydraulic fracturing operations. While I was on site I witness BJ Services pump the frac job for stage 11 of 56 on this well and stage 11 of 54 on permit 3408120722. BJ Services pumped approximately 348000 pounds of sand with approximately 6730 barrels of water at rates as high as 84 barrels a minute during this stage. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0617658325 2017-06-28 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Patterson drilling rig 292 is currently cleaning the hole after reaching a total depth of 8997 feet for their intermediate section. The pad is in good condition and the vegetation on the berms appears healthy. The splash containment on the 500 barrel half-round has been extended to eliminate splashing. This has helped contain most of the mist. However, the fence still has some residue caked on it. I advised Ascent that if the vegetation outside of the berm fence becomes stressed they will have to shut down operations until a better containment solution can be determined. The pad drain valves are closed and do not appear to have any leakage issues. All containment appears to be in good condition and is visibly free of tears and punctures. All hazardous materials are within containment areas.
0906796088 2017-06-21 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Patterson 292 is currently rigging up on this well to begin drilling the intermediate section. The Ascent company man indicated that they should be ready to begin drilling on 6/22 if all goes well. The berm vegetation all appears healthy. The pad drain valves are closed and do not appear to be leaking. The containment that has been set up at this time appears to be in good shape and is visibly free of punctures or tears.
1103408593 2018-06-20 UrbanPreliminary Restoration No Violations On the day of my inspection I found this pad containing three properly labeled producing wells. Well is plumbed to flowline from the tubing as well as the casing hanger. Both flowlines show 1100 psi. Surface annular shows no pressure, intermediate annular shows 1800 psi, tubing shows 1600 psi and production casing shows 1600 psi. Voicemail was left with Dylan Souther (614-202-9738) of Ascent to inform him of the interemediate annular pressure. Production equipment includes three line heaters, three GPUs, and six sand seperators. Pad outslopes are well vegetated and site meets requirements of OAC 1509.072A for preliminary restoration.
1493607864 2017-04-27 Drill / Deepen / Reopen No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and attempted to run 100 feet of 30 inch diameter 118 pound per foot conductor casing. While running casing, they could not get past the second joint. Precision had to ream the hole out with a 42 inch auger down to 60 feet to get past the blockage then ream the rest of the hole out. Once the hole is cleaned out they will attempt to run the casing again.
1493657453 2017-04-27 Drill / Deepen / Reopen No Violations Onsite with Inspector Scott Scheiner to witness cementing of the 30 inch diameter conductor casing. Cement job was not witnessed, see Inspector Scheiners report this date for more information
1493961204 2017-05-05 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1515 feet and set 1509 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1600 sacks of class H cement, 1.19 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 90 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1115 psi for 5 minutes.
