0169425332 2017-10-30 Completion Testing No Violations On the date of my inspection I found the pad being prepared for drill out operations. The frac crew has moved off the pad after completing 62 stages on this well. Ascent estimates the drill out rig will be moving onto the pad within 8 days. Pressure gauges on the surface and intermediate casings read 0 and 700 pounds per square inch respectively. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
0279106444 2017-08-16 Completion Testing No Violations On the date of my inspection I found 9 Energy Services running a cement bond log on permit 34-081-2-0699 with a wireline truck. Great Lake Wellhead Inc. was also onsite installing wellheads. Currently there are 5 frac tanks and a few miscellaneous pieces of equipment onsite that are associated with the operations currently being performed on the pad. The pad berms and the area immediately adjacent to the pad have been recently hydroseeded.
0349134461 2017-03-08 Casing String Witnessed No Violations Today I verified that the remaining annulus and cellar were topped out with ready-mix. Ascent has filled the floor of the cellar with ready-mix, leaving an approximately 18 inch barrier between the cellar floor and soil below. Ascent plans to install the mouse hole after a top hole rig completes the surface string, before a big rig is brought in to complete the remaining strings.
1229049715 2017-12-11 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad has 5 recently stimulated wells, 10 sand separators, 5 gas production units, and 2 shared 500 barrel tanks within a metal dike. Identification has not been placed on the wellheads yet. The wellheads are equipped with christmas tree valves. There are 2 flowlines coming from each wellhead. Currently Express Energy Services and ROC Service Company are onsite rigging up flowback equipment on the 5H, 7H, and 8H. Ascent anticipates flowback operation to begin on 12/12/17.
1488829449 2017-03-06 Casing String Witnessed No Violations Today O-Tex pumped 122 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal to top out the backside of the 24 inch conductor. Cement returned to surface and remained static.
1489901534 2017-03-18 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1439 feet and set 1422 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1420 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 88 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1100 psi for 5 minutes.
1494857726 2017-05-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Patterson drilling rig 284 is currently tripping in the hole to begin drilling the production string and is inspecting drill pipe. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. Containment appears to be in good condition with no apparent holes or tears. All hazardous materials are within containment areas. The area immediately adjacent to the process water loadout had standing water that tested 350 parts per million for chlorides. The Ascent Company Man was notified and indicated that he would have the next water truck vacuum up the standing water.
1505491283 2017-09-15 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on 3 of 5 wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 28 of 62. During the frac job, Halliburton pumped approximately 350000 pounds of sand using approximately 6200 barrels of water. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1517673578 2017-03-03 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A 10 foot mine void was found from 70 to 80 feet, a basket was ran at 55 feet. Allied pumped 93 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal to set the shoe, leaving 10 feet of cement in the conductor. Gravel, bariseal, and calcium chloride were added down the backside of the 24 inch conductor to set the basket. Allied pumped 45 barrels of cement down the backside until cement remained approximately 8 feet below the cellar floor. Ascent will have the cellar topped out with ready mix.

1800293308 2018-04-16 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 5 properly labeled wells that are producing. This well is equipped with christmas tree style wellhead that is much taller than normal. Speaking with Lease Operator, he said that there is no tubing is the well and it is produced through the production casing. Pressure gauges read as follows: Surface 3 psi, Intermediate 961 psi, Production 2537 psi. Production equipment includes two 500 barrel steel tanks within their own steel containment, five line heaters, and five GPUs. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
