0108763659 2016-08-02 Drill / Deepen / Reopen No Violations I am on site today to witness a hydrostatic test on the mine string casing. Prior to my inspection, Alpha Hunter Rig No. 9 drilled a 22 inch diameter hole to a depth of 260 feet and then ran 232 feet of 18.625 inch diameter casing. Inspector Scott Scheiner was on site to witness the cement job for the cementing of the mine string. After speaking to the company man, I was informed that Alpha Hunter Rig No. 9 had tagged cement at a depth of 220 feet. While on site I witnessed the rig drilling to a depth of 270 feet and then loading the well bore with 80 barrel of fresh water. The well bore held hydrostatic fluid levels for 30 minutes indicating that the mine shaft is sealed off. Alpha Hunter will now proceed to drill to set the surface casing.
0537112573 2016-08-01 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Alpha Drilling drilled a 24 inch diameter hole to a depth of 260 feet and set 232 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 980 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. Cement was pumped as a balance job and was tagged at 208 feet, the same depth as the bottom of the mine void. 5 barrels of cement was pumped through approximately 140 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket at approximately 167 feet, after setting for 2 hours, cement was pumped to surface and remained static.

1090306694 2016-08-03 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 13.375 inch surface casing. Alpha Drilling drilled a 17.5 inch diameter hole to a depth of 1,565 feet and set 1,555 feet of 13.375 inch, 61 pound per foot casing. O-Tex established circulation then pumped 1,350 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. Approximately 70 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1,160 psi for 5 minutes.

1475590775 2016-10-04 Drill / Deepen / Reopen No Violations On the date of my inspection I am onsite for a status check. Patterson 292 with Ascent Resources has drilled to total depth for this permit and is cleaning up the hole in preparation for running the 5.5 inch production casing. During my inspection I found all containment to be in good working condition. All possible contaminants are within proper containment. Pad drains remain in the closed position. I did not find any visible spills on the pad.
1768102414 2016-07-30 Casing String Witnessed No Violations I am on site to witness the cementing of the conductor casing. Precision Rat Hole has drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter casing. Alliance Cementing then pumped 275 sacks of class A cement, I witnessed good cement returns at surface. Approximately 10 barrels of cement was left inside the conductor casing.

2109244412 2018-02-23 Preliminary Restoration No Violations On the day of my inspection I found pad containing three properly labeled christmas tree type wellheads. All three wells are in production during my inspection. Outslopes of pad are wel vegetated, lease road is solid, and pad is gated/locked. Well has 1250 psi on flow line gauge, 2000 psi on tubing gauge, 1400 psi on production guage, 500 psi on intermediate gauge, and 250 psi on surface gauge. Prodcution equipment includes three sand seperators, three GPUs, and two 500 barrel steel produced water tanks. Tanks are in metal containment. Production records are up to date in RBDMS. Site meets requirements of ORC 1509.072A for preliminary restoration.
