0808166944 2016-06-24 Casing String Witnessed No Violations I am on site today to witness the cementing of the conductor casing. Express Energy Services used an auger rig to drill a 24 inch diameter hole to a depth of 122 feet then ran 120 feet of 20 inch diameter conductor casing. Express Energy then pumped 230 sacks of Class A 15.6 pound per sack, 1.18 percent yield cement down hole. I witnessed approximately 18 bbl. of good cement returns to surface. 40bbls of fresh water was displaced leaving a 10 foot column of cement inside the conductor casing. 1.5 feet of cement was left in the tin horn.

0942691658 2016-08-30 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 74 drilled a 17.5 inch diameter hole to a depth of 648 feet and set 633 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 581 sacks of class A cement, 1.21 yield and 15.6 pound per gallon. Approximately 31 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 867 psi for 5 minutes.
0973191291 2018-03-26 Preliminary Restoration No Violations On the day of my inspection I found this pad containing two properly labeled wells. Surface and intermediate annular pressures read 45 and 5 psi, respectively. Intermediate annular casing is plumbed with a motor valve and tied into a buried flow line. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tanks, within their own steel containment. Production records are up to date. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration
1619500298 2017-01-05 Completion Testing No Violations On the date of my inspection I found FTS International rigged up onsite. They are onsite to hydraulically fracture both wells on the pad. Operations began on 1/4/17. All equipment is within proper containment.
1675326910 2016-12-08 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with a locked gate. The pad has 2 unidentified newly drilled wells. Five frac tanks, 2 light generator units, and some miscellaneous containment pieces are currently on the pad. The wellhead is equipped with a partial christmas tree valve and is surrounded by a metal fence.
