0786465060 2016-08-25 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 26 inch diameter hole to a depth of 115 feet and ran 120 feet of 20 inch diameter 94 pound per foot conductor casing. Express Energy Services pumped 9 barrels fresh water as a pre-flush, then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.5 pound per sack flow seal. 50 percent excess was pumped and there was good circulation. I witnessed approximately 30 barrels of good cement return to surface, cement maintained a static level. More cement returned to surface than should have. Chesapeake will open the conductor on 8/26 and verify cement levels are deeper than 100 feet. There were no centralizers or flow shoe ran. Note: cement was tagged below 110 feet on 8/26/16, there should be no issues with the cement job.

0902839762 2016-10-25 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 74 drilled a 17.5 inch diameter hole to a depth of 775 feet and set 765 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 715 sacks of class A cement, 1.21 yield and 15.6 pound per gallon. Approximately 32 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 890 psi for 5 minutes.
1489577024 2016-08-28 Drill / Deepen / Reopen No Violations I am on site today to witness the pressure test of a two 20 inch conductors due to the deteriorated appearance of the pipe and questionable cement job on 8/25/16. On 8/27/16 cement was tagged at 111 feet, approximately 4 feet above the bottom of the pipe. The pipe was loaded with approximately 11 barrels of water and 100 pounds per square inch was applied to conductor and held for 5 minutes. No pressure loss.

1902021233 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain four properly labeled producing wells. Three of the four wells on this pad have their intermediate annulus equipped with a motor valve and tied into a buried flow line. This well is the only permit not currently tied into this intermediate flow line. Surface and intermediate annular pressures read 0 and 380 psi, respectively. Production equipment includes four sand seperators, four GPUs, and three 400 barrel steel tanks, within their own metal containment. Site is stable and vegetated and meets requirements of ORC 1509.072A for preliminary restoration.
