They expect to spud the well sometime tonight and drill their 27 hole to 230. If they miss the mine void they are going to pull out of the hole and drill the rest of their surface hole. The mine void is expected at 200 feet roughly.
0493058733 2013-09-30 Drill / Deepen / Reopen No Violations I was on location to check the status of this well. Currently they are running their 5.5 production casing at 5000. They expect to have it run sometime around midnight. They have yet to encounter any tight spots, which were an issue when tripping out of the hole with their drill string.
0499536195 2013-07-24 Administrative Inspection No Violations I contacted the Hess company man to determine the status of drilling this wells conductor. Currently they have TD the hole at 120 and will cement it the following day.
0585592070 2013-08-10 Administrative Inspection No Violations I have had several phone conversations with Hess company man Aaron Trigo on HP290 on whether or not they are going to set a 24 mine string or simply trip out of the hole and air drill the rest of their surface hole to 430. Currently they are at 260 with their 27 bit drilling on fluid. This depth is 48 below the possible mine void. They have had complete returns the entire job and have been drilling very slowly through a very hard formation with no losses. They have talked to their drilling engineers in Houston and now do not see the need to set a mine string because they believe they did not hit the mine, but instead lost returns in the Pittsburgh #8 coal seam where the mine was located. I have contacted MRI 3 Brett Farnsworth and discussed whether or not to allow them to skip the mine string and drill their surface. We decided under the circumstances that the mine string is not needed and that they will be able to set surface without issue. They have been informed to notify me of any issues or lost circulation during the drilling. A 22 air hammer will drill the rest of the surface well and 18 5/8 casing will be set at 430.
0621461823 2013-08-14 Drill / Deepen / Reopen No Violations I was again onsite to witness the cementing of the surface casing for this well. Again once we drilled below 300 feet returns continued to diminish and eventually stop. We were able to get only partial returns but once casing was set in the hole they were completely lost. Hess determined that they would prefer another cement plug to try to seal the loss flow zone and set casing at the permitted minimum depth of 300 feet. The plug job was as follows laydown 18-5/8 casing. Strap and make up 4 stands in the derrick. Trip in the hole to 409. Pump drill pipe capacity, make sure pipe is clear. Test lines to 2500 psi, pump 20 bbls gel spacer, 168 barrels Halcem cement 15.8 ppg, 1.16 yld, 5.08 gal/sk, displace with 4 barrels fresh water, fluid came up conductor to ground level, fluid static. Hess will wait on cement 8 hrs or until wet sample is hard.
0664869622 2013-08-12 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the surface casing but circulation issues made the job infeasible forcing them to set a cement plug in the hole and redrill it to try to establish circulation for cementing. The day went as following : Wash and ream from 264 to 430. Circulate and condition mud . Pump 80 barrels hi vis sweep with 20 ppb lost circulation material (lcm). Attempted to break circulation. Pumping 30 stokes per minute to fill pipe. Pressure increased to 280 psi. Fluid rising in conductor and going to flow line. Pressure dropped from 260 psi to 0 psi. Went and check backside and could not see fluid. Pump 64 barrels in the back side fluid dropped 172. Pump 25 barrels of 20 ppb LCM and chase with 1400 strokes no success. Laydown down 1 joint from 400 to 386. and pick up to 343. Pumped 40 barrels of 30 ppb LCM and chase with 1150 strokes. No success no returns. Laydown 1 joint from 386 to 343 and pick up string to 300. Pump 80 barrels of 30 ppb LCM sweep and displace string with 1200 stokes @ 100 SPM and got partial returns, continued to circulate @ 100 SPM and lost 100 barrels, slowed pumps down to 60 SPM with no returns, losses determined severe, decision made to lay out casing. Hold safety Meeting with franks casing on laying down casing and use Halliburton to pump a cement plug to attempt to drill our and seal up the hole. I will return on next attempt to run and cement casing.

Currently the erosion controls have been installed but there are several spots that are going to be redone. Crews are onsite laying containment for HP 290 that is expecting to come onto the location starting Saturday. They are only planning on drilling one well at this time.

0857427908 2016-08-22 Production Wells No Violations On the date of my inspection I attempted to gain access to the well pad location. The entry point in RBDMS has a pad locked gate. I contacted Ascent Resources. Later in the day, Ashley called me back and indicated that Ascent uses the entry off of CR-10, through the Hunt Club property. I will return at a later date to inspect this well.

They are currently rigging down on their current location and are going to be rigging up on the Smithfield A location. They expect to spud the well sometime this week. I informed them that I will be on location to have a prespud meeting with them, specifically regarding the mine string needed in the area.

They are currently trying to drill it and the bit is constantly sliding off track. I informed them that they could fill the hole with cement and drill it back out in order to stabilize the hole. They expect to be completed with the well mouse hole today.
1047314697 2015-04-07 Production Wells No Violations On the date of inspection I was on site to witness a cement job on another well on the well pad. I found that this well that had been temporarily plugged for safety reasons read 2700psi on the gauge. American Energy Utica consultant Mickey Brewer informed me that AEU representatives had come out at least twice to blow the well down since drilling had commenced on the Nomac 83.

Currently the rig is drilling at 55 and has hit several hard spots. Containment has been cleaned up and the erosion controls are well established at this time. There are no gauges currently on the well so annular pressures could not be documented. They plan on setting 260 of casing to isolate the mine void and then continue drilling a 430 deep surface casing hole. Both will be cemented to surface. They plan on raising the derek today and spudding the well Wednesday.
1376240364 2013-08-09 Drill / Deepen / Reopen No Violations I was onsite to witness the grouting of the entire 27 hole drilled by HP 290 in order to try to seal off any flow zones and allow them to properly set their mine/surface casing. They grouted the entire hole and the loss flow zones did not take any of the cement. They are going to have to wash out some of the cement in the well because they did not want the cement in the open hole in the subsections while drilled ahead.
1416330468 2014-11-18 Drill / Deepen / Reopen No Violations On the date of inspection I found that Precision Rathole was in the process of drilling cellars for new wells that will be drilled on the existing well pad. I spoke with American Energy Utica consultant Matt Miller who informed me that he planned to begin drilling conductors this week.
1432454458 2013-07-25 Casing String Witnessed No Violations I was onsite to witness the cementing of the 30 conductor set to 120 on this location. There were several delays in getting the job started. The welders had some issues getting the casing together and down in the hole. The job was also delayed because a water truck got lost on the way to the location. The job was finally pumped as scheduled and a total of 52 barrels of cement weighed in at 15.6ppg and followed by 92 barrels of displacement. The job had an issue when the trucks ran out of water with 9 barrels of displacement left. They had to transfer water to finish the job. All the water pumped before the cement was lost in a likely shale thief zone. The cement healed the formation and cement was returned to surface. H&P 290 will be coming on location to drill the well after its current well is completed.
1492408333 2013-09-16 Drill / Deepen / Reopen No Violations I was on location to check the status of the drilling and to pick up cement and casing pressure test paperwork. The rig is currently filling the mud tanks and the borehole with 12ppg drilling mud so that can trip in and build their curve.
1513563592 2013-10-07 Drill / Deepen / Reopen No Violations I was on location to check the status of the well now that the drilling rig has left the location. Currently only the oil based mud tanks are on location and are currently being trucked off. Waste Management is on site sucking the cellar out and other pad water. There has been pressure building on the 9 5/8 annulus up to 450psi but currently there are no gauges on the well due to the fact they will be preparing it for the completions crews to come onsite. Those pressures will need to be monitored. There was oil/water on the pad containment but it was not leaving the location or containment and is going to be cleared off before containment is pulled up.
1713179525 2013-12-11 Drill / Deepen / Reopen No Violations I was onsite to check the status of the completions of this well. All stages of the well have been completed and a Key service rig is rigging up to run tubing into the well. They expect to be off location by Monday for which they will turn the well over to production. Erosion control are established and in good condition for the winter. There are no violations at this time.

1768584544 2013-09-13 Drill / Deepen / Reopen No Violations I was onsite to talk with Hess regarding the failure of their casing pressure tests. They have recently run cement bond log. Attempted to test casing with Greens would not exceed 1000 psi, attempt with rig pump down kill line put 2000 psi on casing and holding, Made up bottom hole assembly and directional tools and tripped into hole to 8470. Attempted to test casing with Halliburton, pressure reached 4023 psi & dropped to 2300 psi slowly dropping. Checked all surface equipment did not see any leaks. Called office, Rig down Halliburton. They are currently circulating the hole and trying to plan out their next action. A cement squeeze is the likely next step for operation.

1795948952 2013-10-01 Casing String Witnessed No Violations I was onsite to witness the cementing of the 5.5 production casing. I was able to witness the end of the job. The well circulated during the entire job and full returns were seen. They will wait on cement for 8 hours before setting their slips and preparing the rig to be moved to Belmont County. This will be the only well on the pad at this time.
1821440946 2013-07-22 Drill / Deepen / Reopen No Violations This new Hess location is still under construction but has Butchs Rathole on location to start drilling the conductor for the well. Currently the rig is onsite and set over the hole. They are going to only drill during the day. I talked with company man Kevin Toothman and he expects to set 120 of 30 conductor but expects some very hard formations to appear around 80. They are going to commence drilling and bring in the rest of the equipment tomorrow.

2076017891 2014-11-14 Administrative Inspection No Violations I received a call and e-mail from Andy Ingram, a consultant for American Energy Utica (AEU), regarding setting temporary plugs in a completed well previously owned by Hess. AEU plans to drill additional wells on the existing well pad and elected to set temporary plugs in the well for safety precaution. According to Andy Ingram, two temporary packer plugs in the 2.875 inch 6.5lbs/ft tubing. The bottom plug was set at 8,996 feet (X plug) and can be pumped through in either direction. The top plug was set at 8,942 (XN) and can only be pumped through from top to bottom, it does not allow the well to flow to surface. They installed a VR plug in the back pressure valve. They nippled down the production tree and installed a night cap which is easier to work around.
2088940675 2013-08-08 Administrative Inspection No Violations I was contacted by HP290 because while drilling their surface hole they have lost full returns at 189 feet. At 188 feet is the estimated depth of the abandoned mine in the area. They have not drilled any farther and believe that they can seal the mine/loss flow zone with 9 sack grout. They are going to order in 31 yards of grout and put it down hole to allow for a better drilling surface. They will trip back into the hole and drill the hole again hoping that they have missed and sealed off the mine.
2094374779 2013-09-10 Casing String Witnessed No Violations I was onsite to witness the end of the cementing of the 9 5/8 2nd intermediate casing string. 9 5/8 casing was run to 8511. Hit tight spot, Wash down from 8505 to 8548. Made up landing joint attempt to wash down from 8548 to 8633. Pick up weight 480K, Tried to establish circulation , Packed off. Max pressure put on 2350 psi to break circulation. Circulate @ 8643, Set hanger on well head with 420K. Cement plug did not bump, float did not hold, shut in valves on cement head and woc, 66 blls of cement back to surface. They will run a casing pressure test when the cement sets.
2122816669 2013-08-21 Casing String Witnessed No Violations I was on location to witness the cementing 13 3/8 intermediate casing set to 2046. The job went as planned with p 50 bbl polyflake spacer, 346 bbl BondCem cement, 306 bbl of fresh water displacement (2 bbl over calculated displacement). Bump plug with 1,200 psi @ 04:30 on 8/22/1 Returns were established through the cement job and in total 75 barrels of cement returned to surface. Next casing string will be set at 8500 to cover the queenston formation.
2126343660 2013-08-17 Casing String Witnessed No Violations I was onsite for the 3rd time to attempt to cement eh 18 5/8 surface casing for this well. The job went as planned and as follows circulate and condition mud at 6 barrels per minute. Run 8 joints 18-5/8 casing to 303. Full returns. Tag at 303. Wash down from 303 to 331. Pump 2 barrels, Test 3000 psi, Pump 25 barrels spacer, 21 barrels lead cement 15.6 ppg, , 1.19 yield, 156 barrels tail cement 15.8 ppg, 2.31 yield, drop plug, displace with 90.5 barrels fresh water. Bump plug with 500 psi over circulating pressure to 720 psi. Hold for 5 mines. Got back 90 barrels cement to surface with full returns.
