0203470249 2015-05-05 Production Wells No Violations On the date of inspection I found that the well is equipped with a large Texas style well head. There is no Christmas tree on this well like the other three on the well pad. Zero production has been reported since 2012.
0448198300 2010-03-17 Production Wells No Violations Inspection revealed that the well has recently been placed in production. The drilling pits have been closed and the location graded. The site needs to be seeded and mulched to establish vegetation. I will return to the site later this spring to see if the operator has established vegetation. No violations at this time.

The pad has 3 properly identified producing wells and 1 unidentified well that is shut in. There are also 3 sand separators, 3 vertical separators, 3 glycol dehydration units, and 1 shared 400 barrel tank within a metal dike. The pad is surrounded by healthy vegetation without. The wellhead is surrounded by a metal cage and equipped with a christmas tree valve and is not connected to any production equipment. There is also a secondary glycol dehydration unit and vertical separator tied into the main production line that leaves the pad.

0930271224 2013-07-10 Production Wells No Violations I was onsite to check the status of the production wells on this location. The location has the production facilities completely finished and the wells are being sold into a poly line, causing the wells to be choked back from full production at this time. I also checked the annulus gauges of the wells and this well showed no signs of annular pressures. There are no violations at this time.
0977490518 2014-03-11 Preliminary Restoration No Violations During my inspection I found a christmas tree valve assembly and a wellhead showing 1800 psi on the tubing. The lease road is graveled and the tank battery has the proper I.D. signage on the tank battery. Vegetation has been established and erosion controls are in place. Preliminary restoration has been passed.

1367507536 2013-05-01 Production Wells No Violations This well is located on the same drilling pad as the Hess NAC 4H and 3H-20 wells. This well has a portion of its production equipment shared with those wells. This well has 4 production tanks in a well kept SPCC dike. The wellhead has around 1500psi of pressure on it and the annulus gauges read zero. The wellhead is contained in a very large cage. The lease road is well maintained and the well is properly signed. This well is on a possible plug and abandon list depending whether or not the production is considered good enough to keep the well. There are no violations at this time.

1676299892 2019-03-28 Plug / Plug Back No Violations On the day of my inspection I witnessed Nicks Well Plugging on location to plug well to surface. Cory Lewis from Weatherford was onsite consultant. A cast iron bridge plug was set at 5105. Rig pulled 2.375 inch diameter tubing up to 5080 and pumped a 56 sack plug to 4580. This 11.8 barrel plug was displaced with 17.8 barrels of fresh water. The rig again pulled tubing up to 1938 and pumped a 41 sack plug to 1568. This 8.6 barrel plug was displaced with 6.1 barrels of fresh water. This plug covered Berea and bottom of surface casing. All tubing was pulled and Appalcahian perforated the 5.5 inch diameter casing at 228 feet. No 5.5 was pulled from the well. The rig broke circulation with 20 barrels of fresh water and pumped 17 barrels of cement, bringing cement to surface. Final plug was 81 sacks. The cement manufacturer is cemex. The cement used was a class A neat cement with a weight of 15.6 ppg and yield of 1.18.
