0117545485 | 2019-03-06 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Nicks Well Plugging on location. Cory Lewis from Weatherford is the consultant. Records indicated that bridge plugs were present in the well however none were found. Bottom plug set in the 4.5 inch diameter casing (11.6#/ft) from 7690 to 7290 using 30 sacks of class A neat cement. Cement weighed 15.6 ppg with mudscale and has a yield of 1.18. Cemex is the cement manufacturer. Bottom plug was displaced through 2.375 inch diameter tubing with 28 barrels of water. A second plug was set from 5568 to 5038 using 40 sacks of the same cement. This plug was displaced with 19 barrels of water. Rig is going to trip tubing and run a CBL to decide where, or if, 4.5 inch diameter casing is free. |

Well is not identified but there is an identification sign at the end of the lease road. Lease road is starting to wash out. No production shown since 2013, production records are current. There is no berm or fence around well. I will contact company and see what their plans are for the well. Left voicemail 6/20/18. Well is shut in and has lock out/tag out locks on it preventing it from being opened. |

1349892390 | 2012-10-10 | Production Wells | No Violations | My inspection found the access road to well labeled and maintained. There were two water pumps and some water transfer lines along the side of the access road. The site has a guard shack with an on site guard for security. The pad contained four 400 barrel tanks, gas separators, and an assembled well head. The site around the well was also contained with spill prevention liners. There are no violations at this time. |
1364494988 | 2013-03-28 | Production Wells | No Violations | My inspection found the access road to well labeled and maintained. The well is a plunger lift well set atop a hill above the condensate tanks. The well has production casing with 1500psi and tubing with 300psi on it. A Hess rep told me this is due to the tubing being shut it. Normally both should have the same pressure. There are no violations at this time. |
1376796088 | 2019-03-12 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Nicks well plugging cement the surface plug. Yesterday, 3/11/19 they pumped a 43 sack plug from 4400-4250. The 4.5 inch diameter casing was shot with a jet cutter at 4400 feet but they were unable to pull the casing. The rig pulled 175k on it and got a stretch of 30 inches. CBL shows possible bond 500 feet to surface. They also set a cast iron bridge plug at 1975 feet and perforated with 10 shots. They pumped a 61 sack plug from 1975-1775. Jeff Adams witnessed plug on 3/11. 4.5 inch diameter casing was again perforated at 219 feet. Final plug was set from 219 to surface using 64 sacks of class A neat cement. |
