0615569347 2008-07-09 Drill / Deepen / Reopen No Violations I received a complaint from Mr. Dempsey Re: Corn crop scalded by brine. I took a water sample from the pit. I took pictures of the scald area and took GPS points around the perimeter of the scald area. I took a soil sample from the scald area. The brine appears to have left the pit and went in the ground. The pit is up hill from the corn field and there a seep area that is discolored near the corn field. During the inspection I discovered another seep and scald area. This brine scald came from fluids that were allowed to escape at the wellhead. A valve was open and allowing fluid and gas to escape. The fluid was pooling up at the wellhead. A rain event flowed the contamination down the slope. I took a water sample at the 2nd seep and a 2nd soil sample from the surface scald area. A detailed sketch is attached to the complaint form.

1215697309 2008-07-09 Drill / Deepen / Reopen Violations Noted Received a complaint from Daryl Dempsey on 7-7-08 Re: Brine scald in his corn field. I Inspected the site on 7-9-08 and saw the damaged corn crop located North of the well down slope from the well and pit. I took pictures and GPS on the site. I also took a sample of the pit water and a soil sample of the scald area in the corn field. I then proceeded to inspect the entire location. I found a brine seep located to the South East of the well. A scald area is also located near this seep but it is caused by the run off from the well site. This is the brine that was allowed to blow out from the well and collect on the ground. I took a water sample at the seep and then took a soil sample at the scald site. A third soil sample was taken to the West of the well site to be used as a control in determining the soil condition before the contamination. I called Knox and talked to Bill Mitton about this problem and they are pumping out the pit right now and taking it to Bowers disposal well. They need to remove the contaminated soil and replace it with clean top soil. The pit needs to be closed and any discharge from the well needs to be contained in a tank. I have called Mr. Dempsey and informed him of the status of the investigation at this time.
1230037178 2008-07-14 Drill / Deepen / Reopen No Violations Meeting with Bill Mitton Knox Energy on location 11am. Bill agreed to close the pit and to pay Mr. Dempsey crop damage. Bill said the damage was caused from a mist that escaped from the well not the pit water that escaped. He did agree that the discolored soil at the corn field was from the escaped pit water. He will reclaim the location and apply lime to the site.

Well is not connected to line. 4.5 casing has two 2 ball valves. Gauge on 4.5 annulus reads 550psi. The well is equipped with tubing and a valve. No gauge on the tubing. A steel tank is on location. The tank is made of a half section of a 100 bbl tank. The tank is welded to a skid the top of the tank is open.

1497227228 2017-06-09 Production Wells No Violations On the day of this inspection, I found an oil and gas well equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing with a pumping unit with a gas motor. Tank battery consisted of an orifice chart meter, a drip and a vertical separator with a one 100 barrel steel production tank.
1528000019 2008-06-18 Drill / Deepen / Reopen Violations Noted 2 inch valve off the annular space is open and discharging brine and gas on to the ground. A pool of brine at the well head is 15 feet by 10 feet and 1.5 inches deep. I tested the pool of water and it was off the high end Quantab chart. The 2 inch valve could easily be piped to the pit. Pit location N38.92291/ W082.62688. I called Bob Reel (Equitable) and he said that they are done working on the wells and that I should call Bill Mitton (Knox). I called Bill Mitton and he said that he will be at the location tomorrow. The pit liner has fallen in on one side. I told Bill about this liner problem.

2002402672 2008-09-30 Production Wells No Violations Well leaking gas at last 1.5 collar on tubing string. Where collar and 1.5 by 2 swedge is attached. Swedge is tight but still leaking. Pipe dope is blown out . My gas detector did not detect any explosive levels of gas at wellhead. I was able to hear and see the leak due to the openings in the pipe dope it is blowing through threads. I called Bill Mitton and left a message that wellhead is still leaking. I talked to Mr. Dempsey on location. He said the leak is better today. He is concerned about his pond located below the well site. The cattails have died. He showed me where the spring that feeds the pond is located. I tested the water with a Quantab and found no trace of brine. I called Barry Bowers (Contractor for Knox) he is going to remove the contaminated soil and start on the restoration next week. He is going to call me before he starts.
