0112067892 | 2008-04-16 | Orphan | No Violations | Checked on the status of the well. The landowner had contacted the DMRM stating that the flow rate from this well has increased. During the inspection, it was noted that the flow has not changed since the last inspection. A small amount of oil was observed outside the casing. At time of inspection, fluid was not discharging into nearby intermittent stream. This case was handed over to I/O program. |
0305481833 | 2011-02-08 | Orphan | No Violations | An inspection of this orphan well was conducted with Mr. Andy Thomas. During the inspection, it was noted that fluid was leaking from a 1/4 hole on side of casing. The leak was approximately 1 drip per second. Although no oil discharge was observed, old oil still exists on the ground surface surrounding this well. High Range Quantabs showed 6.6 units, which equal 2378 ppm (mg/L) Cl-. |

0747875715 | 2013-05-15 | Orphan | No Violations | A follow-up inspection was conducted with Orphan Well Program Supervisor, Mr. Gene Chini. A casing caliper utilized for this inspection confirmed 8.5 outside diameter casing. The second string of casing was situated below the top of 8.5 casing and measured 6 in diameter with a tape measure. As previously noted, the 6 casing contained an obstruction near the top with crude oil on the inside and outside of casing. The well also continued to discharge clear fluid from a hole on the side of 8.5 casing. High Range Quantabs were placed in a sample of fluid taken directly from the well and a ditch with standing water located approx 10 away from the well. Both locations showed 5 units which equaled 1111 ppm (mg/L) Cl- (chloride). The US EPA recommended aesthetic value is 250 ppm (mg/L) Cl-. A baseline reading was taken from across the street and revealed 0 ppm (mg/L) Cl-. |
0771981762 | 2014-09-04 | Plug / Plug Back | No Violations | 10:30 AM: Crew arrived on location. The Contractor had searched for the previous 2 days for a 6.25 diameter steel die nipple to attach to the 6.25 diameter casing thought to be in the hole, in order to establish wellhead control. They decided to use an existing collar off of the joint of 6.25 casing that had been pulled from the hole several days prior to extend this casing above groundlevel. Once this is completed they intended to weld 6.25 and 8 casings together to strengthen integrity of the 8 surface casing, which is set to an unknown depth. However, when the crew went in the hole with the adapted 6.25 collar they could not find any more 6.25 casing in the well.
12:00 PM: I arrived at well -out to the top of the Big Lime or next solid plug and stop. 5:00 PM: I left location 5:30 PM: Crew left with plans to cement bottom plug tomorrow. |
0866273324 | 2013-09-05 | Orphan | No Violations | My field inspection this day is with Lall Partap from Fed. EPA program who is the coordinator for the Coast Guard program to plug orphan wells releasing oil into navigatable waters. Today we found casing releasing brinish water and small amount of oil from the casing, flowing down the side of the casing and travelling on the ground to the creek. Lall gathered information for the well and upon return to Detroit, will decide whether well fits into the program. His preliminary opinion was that it will qualify for the Coast Guard plugging. |
0906584350 | 2014-08-26 | Orphan | No Violations | I received a call from inspector Cindy Vandyke. Cindy requested I aid her in the inspection of this orphan well. The site has been finished. A wood matt is in place to support the service rig. 200 barrel tank on site as well as a rubber tire backhoe and a dozer. There has been a well head added to the casing for well control. No work today as the service rig is due 08/27/2014. |

1206726518 | 2008-03-13 | Orphan | No Violations | A follow-up inspection revealed that the fluid level is now leaking from the top of the casing onto the ground surface. Gas bubbling was observed. Quantabs (High Range) placed at the top of the casing revealed 5 units (1601 mg/l). Oil along with a white substance was observed outside the 8 inch above ground casing. Quantabs showed 4.6 units at this location. An oil sheen was also observed in a drainage trench next to this well. This trench is routed to a intermittent stream located 95 ft away from the well. This intermittent stream drains into Queer Creek. An oil sheen was observed inside this trench. Quantabs placed at this location revealed 1601 mg/l. At the time of the inspection, fluid was located approximately 20 feet from the stream. An odor resembling rotten eggs was also noted. This is more likely attributed to the dead organic material inside the casing. Moreover, the PhD Lite Atmospheric Monitor did not show any H2S presence. Since the initial inspection, a permit number has been assigned to this well. |

11:00 AM - I arrived on location 12:00 PM - The crew started drilling this hole with drill pipe and a new 5 diameter tooth bit. Immediately began drilling up wood, then pipe fell several feet and then drilled through a small sand bridge. 1:30 PM - 4:30 PM --grained sand and wood pieces grading to large-grained clean sandstone in the cuttings. The bit will drill hard and jerk, then fall a few feet. No gas present, but well is flowing fresh water which is being directed into the steel tank. I left location. 5:30 PM - Crew left location. |
1319680655 | 2013-01-28 | Orphan | No Violations | Accompanied Brent Bear on inspection for the Idle&Orphan program. Oil was present on the top of the casing. Also accompanied by Justin Smith. |
1327580924 | 2012-01-11 | Orphan | No Violations | Upon arrival, I first noticed that there was a stream of water pouring from the side of the casing. There was also water coming from the top of the 7 inch casing and running down the sides of the pipe. A gas smell was also emanating from the well. Gas was bubbling up from the water. My gas detector, detected a lower explosive level of 2. Upon closer visual inspection it was noted that there was an oil sheen on the water coming from the well. When tested for chlorides with High Range Quantabs, the water had a chloride level of 6.2 which means there are 1807 ppm chlorides in the water. The EPA acceptable value is 250 ppm. Several new photos, and two videos were taken during this inspection. |
1361459403 | 2013-01-28 | Orphan | No Violations | A follow-up inspection was conducted with Mr. Andy Thomas. During the inspection, it was noted that the well consisted of only 8 diameter drive pipe located 4 above ground level with 6diameter and no wellhead. Crude oil was observed 2 from the top on the inside and outside of the 6 diameter casing. In addition, the 6 diameter casing also contained an obstruction that resembled plugging material on both the inside and outside of casing. The 8 diameter casing contained a hole on the side, which allowed for a fluid discharge with oil sheen. Previous inspections have documented gas bubbling, dead organic material inside the casing (rotten egg odor) and Cl- levels within the fluid that ranged from 1601 ppm - 2378 ppm. The well was situated between Goose Creek Road and Goose Creek. An earthen trench extended from the well to Goose Creek located approximately 95 away. An inspection of the creek revealed no oil impact. The property was For Sale at time of inspection and a new Landowner Right of Entry and Plugging Application could eventually be required. Note: This well is located within Columbia Gas Transmission storage field buffer zone. |
1410270633 | 2014-09-09 | Orphan | No Violations | Huffman Bowers has finished plugging the well. The rig has been moved and the well casing has been cut off 30 below grade. The identification plate was placed on top of the casing and the casing was backfilled. The landowner has requested the stone that was used to support the service rig be left. I instructed Barry Bowers to obtain a landowner wavier to allow the stone to be left in lieu of restoration with topsoil and grass seed. I provided a landowner wavier to Barry Bother and asked him to return to myself after it was completed. |
