0722284967 2007-07-18 Preliminary Restoration Violations Noted An inspection revealed that the preliminary restoration has not been performed. Erosion was observed along a steep embankment. Also noted was no vegetation and the pits have not been closed. On 4/11/2007, Mr. Steve Opritza sent Kilbarger Investments Inc., a letter stating that this well was drilled to the Knox Dolomite at total depth of 4260 feet. The permit issued was for drilling and producing only the Clinton or Utica Formations at a total depth of 3800 feet. In addition, stating that the plat approved for this permit was a 20-acre drill unit. Kilbarger is required to submit new plat or plug back this well to at least 4000 feet to meet spacing rerquirements of the approved permit before any treatment or stimulation occurs. The company states that they will be plugging this well back to 4000 feet in August 2007. Once the well is plugged back, the preliminary restoration will be performed.
1187032251 2007-08-13 Administrative Inspection No Violations Spoke this date with Jim Kilbarger re: spacing/acreage problem as a result of this well exceeding the permitted depth. Jim states the well was drilled to 4300 because the driller assumed the well had a 40 acre drilling unit. He has agreed to plug back to 3990. They plan to produce the well at 3960. He was advised to contact the inspector prior to plugging.
1192457337 2007-10-10 Production Wells No Violations An inspection revealed that the company has the well into production. The preliminary restoration still does not meet. The pits have not been closed and no vegetation was observed. Erosion was also observed along the embankment.
1197568079 2007-12-12 Preliminary Restoration No Violations A follow-up inspection revealed that the drill pits have not been filled. The largest pit contains oil and cuttings. The liner contains holes and one side has fallen. This well has been placed into production. Kilbarger states that they would like to keep the pits open because, they plan to frac the well. The site does not contain vegetation and erosion was observed along steep embankment.
1673908831 2007-08-28 Production Wells No Violations Verified total depth of rat hole at 4021 feet. Mr. Steve Opritza has been informed of the inspection results. Kilbarger states that perforations will be above 4000 feet. Records show that 110 sacks of cement was placed around 2.87 inch casing.
