Inspector Andrew Thomas issued a compliance notice on 3/21/2013 for multiple violations. My inspection revealed that the well was not in compliance. The pollution and contamination issues have not been resolved. I did not observe identification at the well or tank battery. The separator has not been moved from the well. The last year for reported commercial production was in 2010. I took two photos of the well. The well owner can not be contacted as he is currently incarcerated. Time of inspection: 9:10 AM. 0144663973 2013-03-21 Production Wells Violations Noted Well is equipped and capable of production. There is a separator 15 ft. from the well head. Identification is incomplete. There is pollution and contamination around the well head as well as buckets of fluid. Uphill from the well is a plastic brine storage tank that is falling in from the top due to compromised integrity from weathering. It is half full of fluid and there is no dike around it. Production records have not been submitted for the year 2011. Separator needs to be moved to an appropriate distance from the well. Complete identification needs posted at the well. All pollution and contaminated fluids and soils need to be removed and disposed of at an EPA approved landfill. The plastic storage tank needs to emptied, contents properly disposed of and removed. Production records need submitted. Failure to comply will result in forfeiture of bond. Accompanied by Southwest Region Supervisor Dan Goins.
