An inspection behind the tank location over hillside to drainage culvert/ravine which drains into a roadside culvert onto Jacobs Road revealed no oil. On opposite side of road located across from tank battery (at lower elevation), an intermittent stream which drains into Sand Run Creek was also inspected. Walked the stream along Jacobs Road and observed no oil or potential seepage. Low flow at time of inspection. Also checked Little Monday, and Sand Run Creek (along Dawley Road to Jacobs Road). At conclusion of inspection, Wayne National Forest representatives Mr. Tommy McDonald and Mr. Gary Craven arrived on-site. Instructed Northwood Energy employees and the Contractor to dig and remove all oil located inside the old cistern. Pumped approx 2 gallons out. Recommend leaving open to allow for any pooling of oil to be removed. Furthermore, completely abandon cistern with clay or concrete once all oil contaminated material has been removed. |

Arrived at location with no daylight and observed fresh oil on ground surface inside 30x10 earthen dike around the tank battery. Approx release at this time 2-3 bbls. Contractor arrived on-site. The battery consists of two (2) 100 bbl tanks and approx 50 bbl saltwater tank. Line with siphoning tube observed broken off (threads) on bottom of 100 bbl. The load line (inside dike) contained an open valve and flat plug removed on end. 50bbl saltwater tank empty with valve open. Flat plugs and valves open on tanks. Behind the 100 bbl tanks, a hole believed to be an old cistern was observed inside the dike. Approx 1 in depth and 5 in diameter and mostly filled in with dirt the observed diameter was 2. Oily soil observed inside. The contractor states that he filled this in 2-3 years ago. Due to the slope, oil has potential to collect in this hole (cistern). Note: The 100 bbl tank located on lease towards well has been bypassed from the well (saltwater tank) and not in use. 2 line still connected and tank appears full. Recommended locks or potentially pump to avoid spill. No observed P&C at the well. The well was not pumping at time of inspection. No offsite impact observed at this time. |
