A field inspection revealed all valves open, and production equipment on site with the AD line connected. The polish rod was not clean and had oxidation on it as if the well hadnt been pumped in a while. Production needs to be submitted to Columbus for 2010. Well needs to be placed back in production, plugged, or placed in temporary inactive status. Failure to do so will result in forfeiture of bond. Spoke with Doug Kitchen on 5/3/2011 at 5:03 PM and he stated that he has bills of sale for gas from this well
0292703506 2009-05-06 Administrative Inspection No Violations Contacted Mr. Paul Nutter today by telephone, 740-347-4643, at 10:38 am regarding the status of this well, as it related to the plugged and abandoned Kepler well location from 1973, Permit No. 777 (FR request by Columbus). Mr. Nutter states that he never owned the Hickman well and that Mr. Rick Mickle is the well owner. Further stating that I would have to contact him for this information. Mr. Nutter proceeded to talk about well ownership. I explained to Mr. Nutter that the well is under his bond therefore, is considered to be the responsible party. Suggested that he talk with Mr. Mickle about a change of ownership form and having him register with the Division.

1244575681 2009-06-09 Production Wells Violations Noted An inspection of the well was conducted with Mr. Robert Stonerock, South Region District Supervisor. The well consists of 4.5 casing with a swedge, regulator and a valve that was observed closed. A 1 black plastic line was observed near the well and found not connected. No tanks were observed during the inspection. The identification placed at the well was not legible and will need to be corrected. The landowner states that natural gas from this well has not been received for a few years. At one time, the gas was utilized to heat their stove. The well owner, Mr. Paul Nutter, had been previously contacted by telephone regarding the status of the well. Mr. Nutter had stated that he never owned this well and that Mr. Rick Mickle was the owner. Currently, the well is under Mr. Nutters bond. Mr. Nutter was instructed to talk with Mr. Mickle about the required change of ownership form. Received a telephone call from Mr. Mickle. He states that this well was producing house gas.
1245684392 2009-05-06 Administrative Inspection No Violations Received call from Mr. Rick Mickle, 740-385-3157 at 12:10 pm. I explained that I had been to the location of Permit No. 777 that had been plugged in 1973 and the final restoration never completed. Therefore, contacted Mr. Nutter to ensure that oil field debris, which includes steel and plastic line was not coming from or related in any way to the Hickman well. Further stating that the Hickman well is currently under his bond He states no. Mr. Mickle confirmed ownership and states that no lines are coming from this well. I explained to Mr. Mickle about well ownership and told him that it would be in his best interest to have the well transferred into his name meanwhile, registering with the Division. Also informed that we could meet and I could provide him with the necessary paperwork. Mr. Mickle said ok and that he was in a hurry because, he needs to get to work. Mr. Mickle replied to question about the status of the well by stating that landowner is receiving house gas.

1459260966 2016-03-29 Production Wells Violations Noted I received a call from landowner Brenda lemon. Brenda stated she had a strip of dead grass in her yard. There is a well owned by Hocking Hills Energy within 20 feet of the dead grass. I called well owner Dough Kitchen and told him about the call I had received. The kill area was caused by a stuffing box leak. The contaminated soil was removed by removing the sod and soil until the soil was free of oil. Peat sorb was applied to the soil as a added measure to ensure no oil remained. The spill was approx.. 16 wide by 30 long. The area will remain unrestored until the lawn drys up. When weather conditions allow, topsoil and seed will be used to fill in the area of soil removal.

1656510726 2010-08-16 Administrative Inspection No Violations Today, Mr. Ron McBride, Chieftain Energy Contractor, returned my call from last week for Doug Kitchen. Mr. McBride left message stating that they will be applying horse manure. Returned call at 5:20 p.m. and stated that contaminated soils were to be removed and this has already been mentioned to Mr. Doug Kitchen. Further stating that cleanup will resemble the Logan Sand location.

1828891074 2010-08-18 Annular Disposal No Violations Witnessed the cleanup of brine contaminated soil(s) today. Chieftain Energy was instructed to remove all contaminated soils from the location. The cleanup began at the impacted location, which had been noted on the initial report, as the second location (closest to tank battery) measuring approx 77 x 11-25 (approx width). As the company excavated the soils, a field test was performed to evaluate Cl- concentration. Approximately 4 - 5 of soils was removed from this location and taken to Mr. Doug Kitchens shop for compost pile. The water table at this location was relatively high, which limited the amount of excavated soil. Furthermore, a Public Water Supply Line was routed through this location along with nearby Southeastern Natural Gas Line. Chieftain Energy placed calls to the respective parties for safety purposes.A 20 x 10 x 4-5 was excavated today. The source of the leak was from existing annular disposal line that was buried. Chieftain Energy repaired this section of line prior to received complaint. The soil was stockpiled today and will be removed tomorrow when, transport equipment arrives at the location.
1864773032 2010-07-23 Annular Disposal Violations Noted A landowner complaint was received alleging brine contaminated soils and dead trees. An inspection revealed two bare areas between the tank battery and the well. Both locations consisted of bare ground and dead/ stressed trees. The source of the release is from the annular disposal line described as black plastic with a diameter of approx 1.25. The line extends along the edge of creek bank and located above impacted areas. No leakage at time of inspection. The trees ranged from young to mature growth and approximately forty may have been impacted from the release. The majority of impacted trees are located along creek bank and next to black plastic. Overall distance to creek approx 50. The first location (closest to well) measured 100 6 (length) x 15 -20 (approx width). The second location (closest to tank battery) measured 77 x 11-25 (approx width). High Range Quantabs were placed at this location. The test was stopped after 7.2 units, which equals 2790 ppm (mg/L) Cl-. Note: The US EPA recommended value is 250 ppm (mg/L) Cl-. A voicemail message was left for Mr. Doug Kitchen, on his cell phone, today at 3:10 p.m. of complaint received, inspection results and time frame of two weeks for cleanup.
1865299603 2010-08-19 Annular Disposal No Violations Returned to the well location today to witness cleanup of brine contaminated soils. The excavation of the second location, as previously noted in the initial report was completed. Chieftain Energy was instructed to apply layer of horse manure and lime prior to placing new soil on excavated site. A second application of lime and manure would be mixed in on top along with seed and mulch. Witnessed application of lime prior to installation of new soil. Witnessed 2 loads of contaminated soil placed into trailer for removal from site. Horse manure will be applied tomorrow along with new soil. The next location referred to as the first location (closest to well), in the initial report, measuring 100 6 (length) x 15 -20 (approx width) will be excavated tomorrow. Only a test hole was excavated today to approx 3. The removal of soil from this location will also be limited by water table, public water line, gas line and nearby septic system. Therefore, only 2 - 3 of soil will be removed. The same procedure performed on second location will also be applied to this location. Note: Mr. Doug Kitchen agreed to a non-compliance for non-related wells today. Mr. Kitchen will provide a letter stating requirements and timelines.
1876014828 2010-08-12 Annular Disposal No Violations A follow-up inspection conducted with Mr. Dan Goins, Mineral Resources Inspector II revealed non-compliance. It was noted that Lime has been applied to the impacted areas. No evidence of tilling, seed, mulch and horse manure. Mr. Doug Kitchen will be contacted regarding the status of well location.
1889348293 2010-02-11 Annular Disposal Violations Noted An inspection of the well and related tank battery revealed illegal Annular Disposal connection, venting of gas, and earthen dike. A 1.25 black plastic line was found connected to the bottom of a 100 bbl saltwater tank. The valve for this tank was observed open. A trenched area behind (2) 100 bbl tanks contained this line (not yet covered) before burial to well. Dug below ground near the well head and observed the AD connection. As noted above, the well now contains (2) 100 bbl tanks with no front wall for earthen dike. A creek is located behind the tanks. At 3:15 pm, Mr. Doug Kitchen was contacted at location, but unable to complete conversation due to bad signal. Left message at 3:45 pm regarding inspection, unrelated NOVs and to return call. Mr. Kitchen returned my call at 4:21 pm and states we cant get nothing bye. Me and Ron just wanted to see if it would take water before testing. Informed Mr. Kitchen that AD would be disconnected tomorrow and agreed to placement of meter instead of flare due to potential gas sales. Note: snow cover at time of inspection.
