0488673118 2008-08-20 Production Wells Violations Noted An inspection revealed contamination and pollution. Fluid was observed inside the dike. A Quantab placed in this fluid exceeded 10 units. Bare areas were observed in two locations, in close proximity to each other, and found to extend over an embankment. Measurements of these areas were 62 x 8 and 53 x 4. Contamination and pollution was also observed on the ground surface around the bottom of old tank (not in use) and parted steel line located, in front of tank. The line appears to have been abandoned. After observing the path of both locations, the source appears to be from the earthen dike/and or drain. Mr. Mike Johnson was contacted by tleephone and informed of the inspection results.

There are two plastic tanks on site that are not being used and are damaged beyond repair. Vegetation is sparse on the site. A phone call was placed on 01/09/13 at 8:44 am to Mr. Mike Johnson about these issues. I left a voicemail. I told him that he should remove the tanks when the site dries up and attempt to establish vegetation in the spring. I also told him that identification must be posted at the well head or tank battery as soon as possible.
