0068689408 2010-08-02 Production Wells No Violations A follow-up inspection revealed non-compliance. The well owner has made progress towards achieving compliance. Lime and mulch have been applied to impacted area outside the dike. Although fertilizer was recommended, it was not observed on ground surface. The inside of this dike will also need to be addressed, due to brine contaminated soils. As previously discussed with well owner, we have agreed to observe 100 bbl tank for potential leakage. If leaking the well owner will replace the tank then perform required cleanup inside the dike.
0247276047 2010-06-01 Production Wells Violations Noted A follow-up inspection revealed non-compliance. No observed identification at the well and has faded at the tank. A bare area measuring approximately 25 x 2-4 with eight (8) dead trees were observed below the earthen dike location. The impacted area begins directly below a 1 black plastic line with a valve on the end (dike drain). High Range Quantabs placed inside the dike exceeded the maximum unit of 10. 8.8 units is equal to 6765 ppm (mg/L) Cl-. The U.S. EPA recommended aesthetic value is 250 ppm (mg/L) Cl-. The brine inside the dike will require pumping. Soils shall be removed from locations outside and inside the earthen dike with application of new soil, lime, seed and mulch. At 2:41pm, an attempt to speak with the well owner by telephone was not successful. No answer therefore a message was left requesting a return call to talk about todays inspection. To date, no return call has been received by DMRM.

1503854473 2010-06-29 Production Wells No Violations Met with the well owner, Mr. Volnee Crabtree and his pumper, Mr. Carl Smith. Mr. Crabtree states that Mr. Crabtree and Mr. Smith were asked why the area below the dike contained stressed vegetation? Mr. Smith states that the fluid level in the dike got too high and didnt realize it was brine. Further stating that water had been fresh in the past, but they had a line leak between the two 100 bbl tanks resulting in release of brine into dike. Ohio Pump & Supply since repaired the line. Walked through the location with Mr. Smith and recommended two options for cleanup of contaminated soils outside the dike: dig and haul; or bioremediation. If bioremediation were selected, horse manure, lime, seed and mulch would be needed. And further emphasized that impacted area may potentially need to be addressed on more than one occasion to ensure vegetative growth has been established. Mr. Crabtree agreed to pump the brine inside the dike into 100 bbl saltwater tank. This will be done to ensure that 100 bbl tank does not contain a leak prior to the removal of soil inside the dike. Mr. Crabtree provided an invoice from 6/15/2010 showing 80 bbl of brine hauled from location to Junction City disposal well. Explained the requirements for well and tank identification. Mr. Crabtree states IDs will be corrected.
