0040871665 2010-10-12 Plug / Plug Back No Violations An inspection revealed that the location has been graded and straw applied. Mr. Ron McBride called yesterday and stated that cement had dropped a total of 11 from surface. He was instructed to place Quickrete in the well and cut casing off 30 below grade and install ID plate. Mr. McBride was at the location and confirmed the above work had been performed. Further stating that 3 sacks of Quickrete were utilized. The landowner,Mr. Paul Stafford, arrived at location and voiced opinion regarding lease road and water well. Mr. Stafford stated that since the o/g well has been plugged, he no longer has gas in water well. An inspection of water well with gas detector showed confirmed this statement. Mr. Stafford and Mr. McBride discussed plan to divert water from lease road. Mr. Stafford would like for lease road to be left and requests gravel. The restoration law was explained to Mr. Stafford. Furthermore, a landowner waiver provided to Mr. Stafford at the location.

0652236357 2008-10-28 Production Wells No Violations An inspection revealed that the well is not in production at this time and missing identification. Valves were observed to be closed on 4.5 casing and tubing. The flowline was found not connected to tank at top of hill. The tank located at bottom of hill was empty. Records indicate no production. The company will be contacted regarding this inspection. The status of this well will be checked at a later date.
0790732874 2010-10-06 Plug / Plug Back No Violations Witnessed only bottom hole plug today. The well owner made an attempt to complete plugging and abandonment today, but was not successful. The plugging contractor struggled with the retrieval of 4.5 casing therefore, only a bottom hole plug was set. A wireline service may be contacted and the cement company will be scheduled for another day. Please refer to detailed activity on plug report.
1123703438 2010-02-04 Annular Disposal No Violations A complaint was received by the DMRM alleging water well disruption at 9525 Bear Run Rd, Logan, OH. The complainant is alleging that natural gas from this oil and gas well contaminated water well 2 years ago. An inspection of the well revealed that valves were closed on the right side of both pumping tee and casing side. A valve on left side at pumping tee was open, but no attached line. The casing valve was found open with an attached drip also containing an open valve and a 3/4 black line. This line is routed to right side of well and tied into a fitting with a closed valve on casing side just before connection into pump tee. From the right side of well, 2 steel is routed to standing knockout location and ties into approx 2 orange plastic. Approx 180 degree bend at knockout with no connection then goes underground. Note: 100 bbl at well location not connected. 2 orange plastic was found cut on hillside above a second 100 bbl located near the road.
1266843489 2010-02-01 Annular Disposal No Violations An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. During the inspection, it was noted that the AD remains disconnected. At the conclusion of the inspection, the landowner, Mr. Paul Stafford expressed concerns regarding non-production, o/g lease, and water well disruption. Mr. Stafford was advised that the well ownership had recently changed to Chieftain Energy. Mr. Stafford attempted to cancel the lease 2 months ago, but was not successful. Mr. Stafford states that his water quality changed 2 years ago and believes there is a presence of natural gas from this well. Mr. Tom Tomastik, Geologist 4, and Mr. Robert Stonerock, South Region Oil & Gas Supervisor, will be contacted regarding this inspection.
1278945123 2010-04-07 Annular Disposal No Violations In response to a water well complaint on this property located at 9525 Bear Run Road, an inspection of this oil and gas well was conducted by Mr. Jeff Fry, West Region Oil and Gas Supervisor, Mr. Tom Tomastik, Geologist, and Mr. Robert Stonerock, South Region Oil and Gas Supervisor. The complainant alleges that natural gas from Permit No. 2333 has entered their water well. During the inspection, it was noted that the well was idle. The well status remains unchanged, as previous inspections have shown idle well. Note: RBDMS shows well transfer to Chieftain Energy on 9/17/2009. The well was equipped with a pumping unit. The valves were closed on the right side of both pumping tee and casing side. A valve on left side at pumping tee was open with no attached line. The casing valve was found open with an attached drip also containing an open valve and a 3/4 black line. This line is routed to right side of well and tied into a fitting with a closed valve on casing side just before connection into pump tee. From the right side of well, 2 steel is routed to standing knockout location and ties into approx 2 orange plastic. Approx 180 degree bend at knockout with no connection then goes underground. Note: 100 bbl at well location not connected. 2 orange plastic was found cut on hillside above a second 100 bbl located near the road. No records in RBDMS exist for oil and/or gas production. There are no records for perforation and stimulation of this well. The Division will continue the investigation into water well disruption before issuing a Notice of Violation (NOV) for idle well. The well status will again be evaluated at a later date. If status remains unchanged, the Division Management will be consulted prior to the issuance of NOV.
1317728916 2011-09-27 Final Restoration No Violations An inspection of the final restoration revealed a piece of electric wire running down the lease road. Water bars were cut into the lease road. There was good vegetation established on the site except for the lease road. Spoke with property owner Mr. Paul Stafford. He stated that he wanted to keep the lease road, and didnt care about the piece of electric wire.
1927317577 2010-07-13 Annular Disposal Violations Noted An inspection of the well revealed the status remains unchanged. The well stands idle and does not meet the new requirements for commercial production under ORC 1509.062 (A)(1)(2). The well contains a pumping unit with belts, motor and no electric. The polish rod liner is cracked and no production lines are tied into tanks. Valves on right side of well were closed. Only casing valve on left open with attached horizontal drip containing open valve and a 3/4 black line routed to right side of well. The line was found tied into a fitting with a closed valve on casing side before the pump tee connection. From this location, 2 steel was routed to standing knockout location not connected, but tied into approx 2 orange plastic. Approx 180 degree bend observed before going underground. The 100 bbl at well location also not connected. 2 orange plastic was found cut on hillside (approx 150) above a second 100 bbl located near Bear Run Rd. No house gas received. Records indicate total depth was reached on 10/14/1981. Currently, there are no statements of production or perforation/stimulation logs that exist in DMRM database. Background: Prior inspections have also revealed that this well was idle. The Division is currently investigating a complaint alleging water well disruption, on the property of said well. Mr. Kitchen has been verbally instructed of well status before and after the 4/7/2010 inspection with Division Management. Mr. Kitchen has previously stated well purchased as package deal and would plug and abandon. Division Management have now ordered the issuance of this Compliance Notice (formerly known as Notice of Violation) for the well to be plugged and abandoned. To date, no plugging permit has been issued by the Division. On 7/12/2010, Mr. Kitchen was verbally instructed to plug and abandon this well by telephone at 10:16 a.m. Mr. Kitchen emphasized his desire to plug and abandon and agreed to 60 day deadline for compliance.
