0413868158 2012-06-04 Production Wells No Violations Brine contamination on site has been remediated. Seed and straw has been applied to those areas as well. Containment dikes have been built around the well head and all tanks. Seed and straw have also been applied to these areas. The poly-tank that once held brine has been removed. Tank battery has been plumbed in a way that brine is piped to the tank closest to the road so that it can be hauled away. New identification has been posted on the tank closest to the road. Update photographs were taken at this site.

I spoke with Mary Schrader, wife of well owner Francis Schrader concerning the clean up of this site. She has called RC Poling to have them clean up the site. I have contacted Dick Poling to confirm that he will be hired to clean up the spill.
1298818162 2012-03-07 Administrative Inspection No Violations Received phone call from Roger Schrader at 8:05 am on Friday 3/7/12. He is the brother of Larry Schrader, and son of Leroy Schrader. He first stated that when his father passed, an attorney was paid to switch everything from his fathers name to the three sons names. This did not happen for the well. I then went over with him what needed to be done to gain compliance with the well. I then gave him the number for the permitting section in Columbus so that he could speak with someone about becoming a registered well owner.
1330606717 2012-02-29 Production Wells Violations Noted The original intent of the inspection conducted on 2/29/12 was to determine whether the annular disposal system on the well was disconnected. I was accompanied by Inspector Kip Thompson. The annular disposal was disconnected and plugged. We then noticed a piece of black, 1 inch plastic pipe that went over the hill. It was connected at the top of the hill to a plastic tank and went down over the hill to a drainage ditch that channeled overflow from the pond to the creek. The water coming out of the end of the 1 inch black plastic pipe was flowing at a rate of .5 Liters per minute (it took 2 minutes to fill the 1 Liter plastic cubitainer for sampling). Water coming from the end of the 1 inch black plastic pipe was tested with a High Range Quantab titrator (300-6000 ppm). It maxed out the Quantab meaning that the chloride content was greater than 6000 ppm. Identification was on the tank. It was incomplete and out of date. The dike around the brine tank and the production tank was insufficient as per SPCC requirements. Mr. Larry Schrader was called at 2:50 pm, notified that there was an issue with his well, and he came directly to the site. The owners of the well need to become registered, and provisions need to be made to get the brine hauled away and disposed of at an approved injection well site. Correct identification needs to placed on the well and dikes need to be built around tanks to meet SPCC requirements. Contaminated materials around well head and creek need to be remediated.

I took water samples from the pond, the end of the 1 inch black plastic, and 100 feet downstream from the end of the 1 inch black plastic. I used a 1 liter cubitainer and a 250 milliliter container at each sampling point. I then asked him how long he had been doing that. He stated that he had probably been doing that a couple of years. He stated that he wasnt aware that he wasnt allowed to get rid of his brine in that manner. I then asked him how long the poly-tank had been there. He stated that his father put it there several years before he passed away. He then told the story of how his father passed away, and he and his two brothers ended up with the property and the well. I then asked if he or his brothers had gotten registered as a domestic owner. He said that he was unaware that he had to
1330950560 2012-03-02 Administrative Inspection No Violations At 11:00 am on March 2 2012, I met with Mr. Larry Schrader and provided him with a copy of the Laws and Rules (1509 ORC and 1501 OAC) as well as a copy of S.B. 165 and a registration packet containing all of the paperwork needed to become a registered well owner. I also provided him with Erica Freemans phone number at Columbus office if he had any questions about getting registered.
