0285240430 2009-09-22 Production Wells No Violations A follow-up inspection revealed that company has been making attempt to pump the well. The flywheel appears to have broke, as it has been removed and will need repaired. At time of the inspection, no leaks observed from 210 bbl tank. Chieftain Energy contractor states that they placed bentonite inside tank to remedy the leak. A 1 plastic line from well is routed to residential dwelling for house gas.
0414227181 2011-08-02 Production Wells Violations Noted Tank battery was full of oil and brine. It had overflown the dike and a plume of vegetation kill was observed 20x25 yards in area. 210 barrel tank seemed to be empty. As noted from a previous inspection, the plastic tank that was in the battery was cracked and had large holes in it. A high range quantab test was ran on the fluid in the dike. It showed 2.6 which is representative of 376 parts per million (milligrams per liter). 250 parts per million (milligrams per liter) is the EPA acceptable value. Further research on the well in RBDMS shows that the well has not been in commercial production since 2007. Doug Kitchen was called immediately after inspection and a voicemail was left on his phone. Ron McBride was also called and notified of the situation. Well needs to be produced, plugged or placed in temporary inactive status. Property owner does receive house gas.
0497749856 2009-09-09 Administrative Inspection No Violations Mr. Ron McBride called today at 10:25 am and stated that Chieftain Energy has been pumping this well since last week.
0861029983 2011-08-04 Production Wells No Violations Witnessed clean-up efforts performed by Ron McBride. Contaminated material was hauled away by Scott Courter. Material was hauled to Chieftain Energys shop. Did chloride test for the soil in the field. It passed. Fluid was sucked out of battery. Tanks were removed from battery. Contaminated material was removed from dike area, and surrounding vegetation kill area, then filled in with good dirt. Areas will be seeded and strawed.
1034766987 2008-12-31 Production Wells No Violations A complaint was received alleging non-production. The complainant states that the company has not pumped the well for a few years. An inspection revealed that the well contains a pump jack and shows signs of pumping (ie polish rod). The complainant states that recently he has pumped the well himself. Valves were observed open with attached flowline. The company will be contacted regarding the well status.
1240587779 2009-04-01 Production Wells Violations Noted A complaint had been received alleging non-production and that house gas has not been received for 2 years. Further stating that the company had pumped the well for 3 months and then walked away. Spoke with Chieftain Energy contractor following the previous inspection, in regards to complaint and inspection results. Mr. McBride confirmed non-production and that the well had not been pumping for a few years. He was instructed to have this well placed back into production or plugged. Also mentioned was that the dike walls would need to be built up. A follow-up inspection revealed that the well is still idle. The well contains a pumping unit with 2 plastic flowline routed from the well to a 210 bbl tank. A section of the dike around the tank was found cut along the side. The well head does not contain a dike.
1304518233 2011-05-02 Production Wells Violations Noted No production since 2007. Production not submitted for 2010. A field inspection revealed that the well is equipped for production, there is no identification at the tank battery or well head. The poly tank in the tank battery was cracked all the way around and had holes in it. There was standing water with residual oil on top of it in the dike area. Horse manure was also present from a clean-up effort. The polish rod had vines growing on it. Gas was being vented from the well head. Identification needs to be placed at the well. All fluids need to be removed from dike area. Venting of gas needs to cease or gas needs to be flared. Production needs to be submitted to Columbus for 2010. If the well is not producing or capable of producing, well must be plugged or placed in temporary inactive status. Failure to do so will result in forfeiture of bond. Spoke with Doug Kitchen at 5:03 PM on 5/3/2011 regarding the venting of gas and production status of the well.
