1304695171 | 2011-05-06 | Production Wells | No Violations | At 9:50 AM on 5/6/2011 I arrived at the tank battery for the Armstrong lease. It was observed that the dike was still at least if not more than 75% full of fluid. At 10:00 AM on the same day, I called Mr. Doug Kitchen and left a voicemail for him telling him that the dike didnt seem like it had been pumped out. He called me back 5 minutes later and handed the phone to Ron McBride. Ron stated that 80 barrels of water was sucked out of the dike. I then called Kilbargers and asked if the water truck driver could call me back. At 10:20 on 5/6/2011 I received a call from Larry Howdyshell, the water truck driver for Kilbargers. He stated that he sucked a load of fluid from the bottom of the dike and pumped it into the 210 barrel tank that is in the same dike that encloses that tank battery as per Chieftain Energys instructions. Note: the 210 barrel tank looks like the 100 barrel tank sitting next to it because it has sunk into the mud so far. |
1648144431 | 2011-05-03 | Production Wells | Violations Noted | At 4:15 PM on 5/3/2011 I was driving by the tank battery and noticed it overflowing. I stopped and took a water sample from the bottom of the dike. The low range Quantab results were a reading of 4.8 (231 ppm(mg/L)). There was an oil sheen on top of the water. Spoke with Doug at 5:03 PM on 5/3/2011 regarding this matter and others and he said that he will have the dike pumped out on 5/4/2011. |
1734437269 | 2011-03-29 | Production Wells | Violations Noted | On Tuesday 3/29/2011, when driving on Maysville-Williams Rd. with Mike Sherron from OEPA, it was noticed that one of the tanks in the tank battery had overflowed. Upon closer inspection it was noted that oil and brine had escaped the earthen dike and was flowing in a path down a lease road towards the creek. Pictures were taken, and Doug Kitchen was notified. Mike Sherron and I met him on site and discussed remedial action. Mr. Kitchen said that he would have the dike pumped out, and have a small track-hoe and a dump truck on site the next morning to remove contaminated soils. |
