0492059631 2009-05-15 Annular Disposal No Violations A follow-up inspection was conducted with the well owner, Mr. Paul Sinkovitz. During the inspection, it was noted that two rubber lines that were attached to a steel line routed behind well head and between pumping unit was cut. This line was a source for the disposal of brine back into the casing. Facing the well head, the steel line contained a valve, on the left hand side, and was observed closed. with a bucket placed underneath. Inside the steel line at the valve location, salt crystals were observed with bucket placed underneath this line. The lines were found still attached to the tank. Recommended that a plug be placed inside the cut lines. As discharge of fluid onto ground surface was observed while the well was pumping on 5/13/2009 from nylon line. High Range Quantabs placed in fluid at the well head location revealed 9.2 units.

0780727201 2013-03-21 Production Wells No Violations No attempt has been made by the operator to gain compliance. The ditch with oily fluid still has oily fluid in it, the pollution and contamination around the well head and inside the tank battery remains untouched and the connections from the brine tank to the 4.5 in. annulus and production tubing are still intact. There is also a separator that is too close to the well head, the identification tag is incomplete, there is a plastic storage tank full of fluid 50 ft. away from the well, and the tank battery is 30 ft from the well. The minimum requirement is 50 ft. I was accompanied by Southwest Region Supervisor Dan Goins and Inspector Kip Thompson.
0807757685 2006-08-18 Annular Disposal Violations Noted An inspection was conducted at the facility owned by Paul Sinkovitz, on 8/18/2006. During the inspection, it was noted that the dike was full of fluid and needs to be pumped and oily material removed. Additionally, the culvert contained brine which also needs to be removed.
1075828563 2013-01-31 Production Wells Violations Noted Accompanied Jon Scott on investigation of suspicious plumbing on this well. New photos were taken by Jon. There has been no attempt by the operator to gain compliance. We established that the 2 line connected to the annulus is being used to dispose of brine water. It is also manifolded in a way to allow the flow of brine into the production tubing. There is a check valve on the tubing connection that allows flow into the tubing and disallowing flow from the tubing. All brine lines coming from the tank battery to the well head need to be removed.
1112371052 2008-08-04 Production Wells Violations Noted An inspection revealed contamination and pollution at two locations. The first location was inside a trench located behind the well. The second location was inside the earthen dike.
1241715950 2009-04-23 Production Wells Violations Noted An inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock and Mr. Paul Sinkovitz. It was noted that Mr. Paul Sinkovitz is placing brine back back through the casing into the well bore. Mr. Sinkovitz stated that he floats the oil out, which also alllows for him to continuously pump his well. Mr. Tom Tomastik, Geologist, Technical Support Services Section and Mr. Jeff Fry, UIC Field Supervisor were then contacted and stated that this operation is considered to be illegal saltwater injection. On 5/1/2009, an inspection conducted by Mr. Stonerock and Mr. Fry confirmed illegal saltwater injection Both Mr. Stonerock and Mr. Fry informed Mr. Sinkovitz, in the field, to disconnect lines to the well for this operation.
