0145263011 2019-01-09 Production Wells No Violations I conducted an inspection on this well today accompanied by landowner Barney Cook. My inspection revealed that the well was equipped with a pumping unit but no motor. The well was not capable of production. This well produces into a shared tank battery with API permit number 1157. The tank battery consists of (2) 100 barrel tanks and a knockout tank inside an earthen dike. Identification was posted at the tank battery but not at the well. The last year for reported commercial production was 2014 in which Hocking Hills Energy and Well Service reported 18 barrels of oil and 75 mcf of gas. I took four photos of the well. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 9:05 AM. Weather: 28 degrees F, Snowing

1076793290 2010-03-01 Production Wells Violations Noted A follow-up inspection revealed that the well has been placed back into production. The well is equipped with a pumping unit and contains production equipment at the well head. The well is tied into a nearby 100 bbl tank. At the well location, a 12 x 12 area contained fresh oil from a stuffing box leak. The well was also observed to be venting gas. Notifed Mr. Doug Kitchen today at 8:40 am at the location. Mr. Kitchen was instructed to remove P&C and to not vent gas. Mr. Kitchen plans to eventually sell gas from this well. He states that they knew about the P&C because, the stuffing box rubbers were bad. Note: snow cover at time of inspection. Remove all contaminated material from the well location. The well shall be equipped to prevent the escape of oil. Replace the stuffing box rubbers. Do not vent gas without the use of a flare.
1154299273 2006-07-19 Production Wells Violations Noted An inspection was conducted at the facility owned by Chieftain Energy Corp on 7/19/2006. During the inspection, it was noted that the pump jack was missing a cylinder head and proper identification. It was also noted that gas was leaking from the wellhead and the well appeared to be idle. The polishing rod was rusted, valves were closed and the gas hose feeding to the engine was not hooked up. The area surrounding the tank battery showed signs of a spill, whch may have affected a nearby creek. At the time of the inspection, oil was not observed upstream nor downstream of the spill. It was noted however, that a dike has been built. The ID located at the tank battery must be corrected and show the correct area code and permit number.

On 8/17/2010, at 5:48 p.m., a Remedial Restoration of the well site was recommended to Mr. Doug Kitchen, due to the lack of vegetation and observed erosion from equipment ( service rig and removal of 100 bbl tank). Mr. Kitchen agreed to perform the work and also states that 3 bbl oil now exists at well head and will need cleaned up.

1967679252 2009-03-18 Production Wells Violations Noted A complaint was received alleging non-production and flowline burial. An inspection revealed that the well is idle. The well contains a pump jack, but the bridle was found to be disconnected. The flowline from this well was found connected to a nearby 100 bbl tank with no knockout tank. The tank appeared to be empty. Plastic flowline from the casing side of the well was observed cut in two locations. The first location (near the tank) is parted next to a steel line, which may have been tied together at one time and routed through the woods. The second location was found on top of hill near a separator. Chieftain Energy obtained ownership of this well in 2001 from Mr. Paul Grim. From 2002 to 2005, RBDMS records indicate that there was no oil or gas production. Prior to 2002, the last year for production was 3 bbls of oil in 1996. In 2006, Chieftain Energy reported gas production (same figure for permit #1157). Currently, no statement of production of oil, gas and brine has been submitted for 2007 and 2008 (ORC 1509.11). Permit #1157 is routed to (2) 100 bbl tanks near the road. A meter located near the tank battery contained a chart (no sign of production, which stated February 2009. A valve was observed closed on the gas line routed to a separator at this location. The landowner states that this well has not been pumped in a few years. Identification was not observed and will need to be provided. The majority of the flowline from this well is routed through a wooded area and placed along a hillside. Therefore, the burial of flowline will not be required at these locations. Plug or produce this well. The bridle is disconnected and the flowline will need to be repaired.
