The well is being used for domestic gas use. The dike surrounding the 2-100 barrel tanks has failed in one spot from erosion. I called and left a message for well owner Mrs. Marty Weis at 10:18 this AM.

0610993782 2010-09-21 Production Wells Violations Noted The Division received a complaint alleging that odor from well is causing a nuisance. An inspection of this well revealed gas is being vented from the well without the use of a flare. The well owner will be required to install a flare and shall meet the required setback distance of 100 from the well, tanks and existing inhabited structure. The flare shall be positioned so that no gas condensate is drained onto public road or structures, which could cause a safety hazard. The well is located in Falls Township, which is considered to be an Urban area. The well owner shall install on tank(s) activated charcoal filters on the vent stack and replace filters as needed. Vent stacks shall have functioning low-pressure relief. Tank hatch lids shall have a functioning seal and the hatch shall be sercured at all times the well owner is not on-site. Each oil storage tank shall have a functioning lightning arrestor. Identification was not observed at the tank and found incomplete at the well. The well owners shall provide legible identification, in 2 inch or larger letters at all times to show: Well owners name; County and Township; State Permit Number, Lease Name and Well Number; Local Emergency Response Phone Number and Company Emergency Response Phone Number. The identification shall be placed on metal or wooden sign and can be attached to a post securely placed in the ground at well or tank battery location. The earthen dike surrounding the tank battery contains a trench cut towards the front. It is recommended that well the owner fill in this trench to ensure containment, if an oil spill were to occur.

0698291618 2019-04-17 Production Wells No Violations My inspection revealed that the trenches that were dug to contain oil have been backfilled. I observed a 100 barrel tank and 2100 gallon plastic tank within an earthen dike. The plastic tank that was used to pump oil into from the trench was still sitting next to where the trench was dug and was full of fluid. I contacted Marti Weis during the inspection and advised her that the tank needs to be emptied. I also advised Ms. Weiss that identification needs posted at the tank battery. She told me she is going to paint the 100 barrel tank and will post identification after the tank is painted. Ms. Weis said she should have the plastic tank emptied and identification posted within 2 weeks. The well and tank battery are now in compliance. Time of inspection: 1:00 PM. 1224367238 2018-09-27 Production Wells No Violations My inspection revealed that the well was equipped with a pumping unit and motor. The tank battery consists of (2) 100 barrel tanks and two 55 gallon drum knock out tanks. The dike surrounding the tanks would not be capable of containing the capacity of the tanks. During the inspection I observed that one of the 100 barrel tanks was leaking brine from the very bottom of the tank. It appeared that the leak had been ongoing for some time as I observed the leak to be a slow leak. The brine leak has breached the dike and impacted the ground surface just below the tank battery. The area that was affected was approximately 30 feet by 15 feet in size. I took several photos of the tank battery area. I was contacted by the well owner Marti Weis at 2:43 PM after I left a voice message. I informed Mrs. Weiss of my findings and advised her that the tank needs to be replaced or repaired and the contamination remediated. Mrs. Weiss said she would contact Mr. Scott Corder today to work on the issues at the tank battery. I asked Mrs. Weiss to let me know when Mr. Corder will be doing the work.
