0932530298 2013-09-30 Orphan No Violations On 9/28/13 the Contractor checked well pressure - 3 PSI on gauge. On 9/30/13, while I was on-site, Contractor tagged the top of cement in the well bore at 495 and water at 100 . Contractor gravity fed 2 bags of oats (for lost corculation) and 84 sacks of concrete mix into the well bore. Tagged top of cement at 395. Ordered 9 yards of 9 Sack Grout and used 8 yards of this to fill the well bore to surface. Shut in well, gas tight, overnite and will check in morning.

1353401603 2013-10-08 Final Restoration No Violations This well was plugged on 9/30/13 and checked for gas leakage on 10/1/13 (no leakage detected).

On todays inspection I checked the site for restoration and found the area leveled and vegetation established.

Final Restoration Passed and Orphan well case closed.
1380538481 2013-09-27 Plug / Plug Back No Violations Washed 2-3/8 tubing to old total depth of 904. Continued washing an finally broke thru and washed down to 945. Hooked up Petroset Cementing and pumped 8 bbls of gel with hulls LCM ahead and 160 sacks of Class A cement and displaced to 400. Shut down for 1.5 hours. (a total of 3 hrs. before the 2nd plug was pumped) Tagged cement at 703 (wireline). The fluid level was ~ 100. Pumped 220 sacks of Class A cement with hulls LCM in the front end and displaced to 130. Pulled the tubing. The fluid level was ~ 60. The well started making gas immediately. The well was shut in.
1382108092 2013-10-01 Orphan No Violations On my inspection today I checked the well for gas leakage with my Sperian Gas Meter. No gas was detected. There was water on top of the cement, inside the well casing. No gas bubbles were visually detected. I will check site in 7 - 10 days for restoration progress.
