0096814769 2019-05-14 Completion Testing No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. A Profrac frac crew is on location tied into the 8H, 10H, and 210H wells. Stage 50/100 is completed on this well. Plug set at 15978, with bottom perforation at 15967 and top perforation at 15851. Stage design calls for 330000 lbs of sand and approximately 10000 barrels of fluid. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 10 psi. There is a bad valve and they are not pumping on this well util it is changed out.

1428153201 2019-05-24 Completion Testing No Violations On the day of my inspection I witnessed a Profrac frac crew onsite tied into the 10H and 210H wells. This well has not been pumped on since my last inspection as the bad valve has not been changed out. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 20 psi. Fourteen steel produced water tanks are on location within their own containment.
1548729754 2019-01-28 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 655 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming Casing ran 670 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot, with 5 foot stick up above kelly bushing. Casing was centralized with 15 centralizers. Shoe track is 44 feet long. Halliburton was onsite with 671 sacks of class H cement. Cement weighed with mudscale at 17 ppg and yield of 1.071. 50% excess ran in open hole. Cement displaced with 97 barrels of fresh water. 30 barrels of gel and 10 barrels of water pumped ahead of cement. 30 barrels of cement returned to surface, and floats held with 1.5 barrels back. All depths are from ground level and kelly bushing is 25 feet.
1548940224 2019-01-30 Drill / Deepen / Reopen No Violations I am on location to witness the cementing of the Intermediate String. I checked in with Joe Foerste (Encino) and John Melville (Encino). Haliburton is on location. The plug was released at ~ 11:41a and it landed ~ 12:09p. Cement began to circulate to surface at 130 barrels of displacement. A sample of the cement returns were taken at 140 barrels of displacement and weighed at 14.5 lbs.
1552585353 2019-03-14 Drill / Deepen / Reopen No Violations Upon my arrival on location, I witnessed the rig crew getting ready to run the 5.5 inch diameter production casing. The rig drilled to the depth of 23,486 feet. They will be ready to cement production casing tomorrow night. The rig will then move off location to another well pad.

1782432131 2019-05-06 Completion Testing No Violations On the day of my inspection I witnessed a Profrac frac crew onsite. Profrac is currently tied into this well and the 210H well. During my inspection, Profrac is pumping stage 34/101. Stage 34 plug set at 18398 feet, with bottom perforation at 18387 feet and top perforation at 18267 feet. Stage design calls for approximately 10000 barrels of fluid, 198000 lbs of 100 mesh sand, 132000 lbs of 40/70 mesh sand, and 3000 gallons 7.5% HCl. Company men said that they are pumping approximately 10% of the stage volume with produced water. There are fourteen 350 barrel produced water tanks on site, within their own containment. Breakdown pressure was 6931 psi. The 13.375x9.625 annular pressure was 0 psi and the 9.625x5.5 annular pressure was 5 psi.
1941160207 2019-06-05 Completion Testing No Violations On the day of my inspection I was accompanied by Supervisor Eric Conrad. During the inspection we witnessed a Profrac frac crew onsite. Stage 56/101 had been perforated but not fracked. Stage 56 plug set at 15076 feet, with bottom perforation at 15065 feet and top perforation at 14945 feet. The 13.375x9.625 annular pressure was zero and the 9.625x5.5 annular pressure is 44. Casing pressure is 5014 psi.
