0101083173 2018-09-26 Casing String Witnessed No Violations On the day of my inspection I witnessed the end of the first stage and the entire second stage of the 9.625 inch diameter intermediate cement job. Xtreme 803 drilled a 12.25 inch diameter hole to 7305 feet. Power Torque services set casing at 7294 feet. Casing is L-80, 40#/ft. Shoe track is 88 feet long. DV tool is at 3794. Casing was centralized with 162 centralizers. For the first stage Schlumberger had 182 barrels of lead cement with a weight of 12 ppg and yield of 1.79. Tail cement was 93 barrels of 15.6 ppg and a yield of 1.19. 35% excess ran on first stage. 40 barrels of 11ppg mud push was pumped ahead of lead cement. First stage displaced with 548 barrels. Rig circulated on well after DV tool opened. 632 barrels of cement to surface. Second stage pumped 263 barrels of cement weighing 15.6 ppg and 1.19 yield. 30% excess on second stage. Displacement was 587 barrels of displacement. 10 good barrels of cement to surface with 50 barrels of cement interface.
1109648033 2018-12-04 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. Identified myself to company men Malcolm Trahan and Bill Pugh. Wireline was just ran on stage 10/85 on this well. Plug was set at 20164, with bottom perforation at 20153 and top perforation at 20033. Surface annular pressure is 300 and intermediate pressure is 0.
1372396995 2018-11-16 Completion Testing No Violations On the day of my inspection I found a Universal frac crew on location. They are zipper fracking the 1 and 5 wells. They are not tied in on this well. Surface annular pressure is 300 and intermediate annular presure is 0.
1454567101 2019-01-14 Completion Testing No Violations On the day of my inspection I witnessed Fortis Rig 1069 on location. They have not started drill out on this well yet. Surface annular pressure is 250 and intermediate is 0.
1524008898 2018-04-17 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1290 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1280.5 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot, with a 3.5 foot stick up. Casing was centralized with 11 centralizers. Shoe track is 43 feet long. C&J/O-tex was onsite with 1340 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 187 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 10 barrels of cement returned to surface, and floats held with 1.0 barrels back. Company man John, cement guru Mike, and cement supervisor Chris Kasas onsite.
1537373656 2018-09-19 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 has skidded over this well and is nippling up. Identified myself to Company man Van Dietrich.

They will start stimulating this well tomorrow. Surface annular pressure was 0 and Intermediate annular pressure was 350.
1834197146 2018-10-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 drilling their 8.5 inch diameter lateral at 17075 feet. Estimated total depth is 21795. Mud weight is 10.8 with no flare. 2000-4000 units of gas. Walk around location revealed what appeared to be oil base mud inside of containment. Company man Matt Miller was told to get it removed prior to rain expected on later in week. He said that he would have rig hands suck it up today.
1902629328 2018-03-28 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augered a 36 inch diameter hole to 100 feet and set 100 feet of 30 inch diameter conductor casing, 118 pounds per foot. CJ/otex was onsite with 330 sacks of Class A cement. Cement weighed 15.6 with pressurized mudscale and a yield of 1.2. 50% excess ran in open hole. Cement displaced with 71 barrels of fresh water to 85 feet. 20 barrels of cement returned to surface. Cement supervisor was Kris Casas (Chili) and Ascent company men Tony Shepard and Sean Fox were onsite.
2063504470 2018-12-20 Completion Testing No Violations On the day of my inspection I was accompanied by Geologist Tammy Richards. Universal frac crew is on location. Operations are down for maintenance and the company men could not be located during my inspection. Surface annular pressure is 300 and intermediate is 0.
