0556497996 2018-12-04 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. Identified myself to company men Malcolm Trahan and Bill Pugh. Stimulation was completed on this well on 11-29-18. There were 86 stages completed. Kill plug set at 7340 Surface annular pressure is 400 and intermediate pressure is 350.
0842175020 2018-08-13 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 drilling a 12.25 inch diameter hole on air. They are at 5960 feet on an 11 degree tangent. Hole is not making much water, if any. A small flare was burning after connection was made. ID to company man Matt Miller. Containment is full of water and near overtopping in the southwest corner. Rain is expected in the near future. Company man was asked to get it sucked down. He said a truck had just pulled onto location and he would direct them to do so.
1022405204 2018-04-15 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1285 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1274 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot, with a 3 foot stick up. Casing was centralized with 11 centralizers. Shoe track is 43 feet long. C&J/O-tex was onsite with 1340 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 187 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 65 barrels of cement (15.6 ppg) returned to surface, and floats held with 1.0 barrels back. Company man Randy Shepard and cement supervisor Drew Forsyth onsite.
1074307082 2018-11-16 Completion Testing No Violations On the day of my inspection I found a Universal frac crew on location. They are zipper fracking the 1 and 5 wells. They just finished to frac on stage 39/26 on this well. Surface annular pressure is 400 and intermediate annular pressure is 0. Stage 40 plug set at 15444, with bottom perforation at 15434 and top perforation at 15313.

Operations are down for maintenance and the company men could not be located during my inspection. Surface annular pressure is40 0 and intermediate is 300.
1693481834 2018-08-21 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 drilling an 8.5 inch diameter hole at 11206 feet. ID to company man Van Dietrich. Kick off point was 7468 and landing point was 8617. there is no flare currently and they are drilling with 11 ppg oil base mud. They cemented the 61#/ft 9.625 inch diameter casing over the weekend. Casing shoe at 7289.
2079699842 2018-03-29 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augered a 36 inch diameter hole to 100 feet and set 100 feet of 30 inch diameter conductor casing, 118 pounds per foot. CJ/otex was onsite with 330 sacks of Class A cement. Cement weighed 15.6 with pressurized mudscale and a yield of 1.2. 50% excess ran in open hole. Cement displaced with 71 barrels of fresh water to 85 feet. 20 barrels of cement returned to surface. Cement supervisor was Kris Casas (Chili) and Ascent company men Tony Shepard and Sean Fox were onsite.
2129682959 2018-11-27 Completion Testing No Violations On the day of my inspection I witnessed Universal frac crew on location. Identified myself to company man Malcolm Trahan and Bill Pugh. They will be stimulating stage 84/86. Plug was set at 8800, with bottom perforation at 8790 and top perforation at 8669. Surface annular pressure was 0 and Intermediate annular pressure was 400.
