0056091876 2018-03-19 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 4 properly labled wellheads. Surface casing annular pressure is 1 and the intermediate casing annular pressure is 11. Production equipment includes six 400 barrel steel tanks inside steel containment, a vapor recovery system, combustor, two compressors, four gas production units and four sand seperators. Site meets the requirements of ORC 1509.072A for preliminary restoration. A hydraulic line blew on a tanker truck today on location. A skid steer arrived on location during my inspection and removed the contaminated gravel. Gravel was already shoveled into piles by hand tools. Estimated 3 gallons of hydraulic oil spilled.

1491562379 2017-04-06 Casing String Witnessed No Violations On location to witness the cementing of the surface casing. 13-3/8 61# casing with all the required float equipment was run in a 17.5 hole to 1305. Halliburton cemented it with 1225 sacks (270.5 bbls) of Neocem cement ( a propriatary blend of Class H and some additives). The cement was displaced with 192 bbls of fresh water. The plug landed at 491 psig and was bumped to 1031 psig for 5 minutes with no drop in pressure. The pressure was released to the truck and 1 bbl of fresh water was returned. The floats held. 42 bbls of good cement were returned to the surface.

1654699515 2017-04-12 Drill / Deepen / Reopen No Violations On the day of my inspection, accompanied by Inspector Randy Akins, we found the Nomac 77 rig drilling at 7727 feet into the curve. Check of the perimeter and containment found no deficiencies . Pad drains are shut in and containing a small amount of water in the drain basins. No violations noted.
