0669306936 2018-05-07 Completion Testing No Violations On the day of my inspection I found a halliburton frac crew rigged up on the 201H and this well. This well is at stage 63 of 75. During my inspection, I witnessed frac stage 63 of 75 on the 201H. 9.625 inch diameter annular pressure is 91 psi.
0989598710 2018-03-12 Preliminary Restoration No Violations On the day of my inspection I found the pad to include 5 wells. During my inspection the wells were having their wellheads built. Patterson 580 moved off of the pad last week and these wells have not been stimulated. The pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
1513743316 2017-12-19 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 580 drilled a 17.5 inch diameter open hole to a depth of 532 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming Casing ran 522 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Casing was centralized with 12 centralizers. Shoe track is 43 feet long. Schlumberger was onsite with 485 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 74 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 35 barrels of cement returned to surface, and floats held with 0.5 barrels back.

Wyoming Casing ran 2057 feet of 9.625 inch diameter casing, j-55, 40 lbs/foot. Casing was centralized with 48 centralizers. Shoe track is 45 feet long. Schlumberger was onsite with754 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 42% excess ran in open hole. Cement displaced with 152 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 40 barrels of cement returned to surface, and floats held with 1 barrel back.
1760917530 2017-12-28 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 drilling an 8.5 inch diameter hole at 16,995 feet. Company man Jackie Holik stated that they are hoping to reach total depth tomorrow and cement 5.5 inch diameter production casing on 12/30. No issues with containment noted during this inspection.
