0034295818 2017-11-07 Drill / Deepen / Reopen No Violations On the day of my inspection I found patterson 580 tripping back in the hole at 2064 feet with their 8.75 inch diameter bit. I was informed they just finished running their 9.625 inch diameter CBL. Containment appears to be functioning properly and may need sucked out in the next couple days if rain persists.
0287412039 2018-03-12 Preliminary Restoration No Violations On the day of my inspection I found the pad to include 5 wells. During my inspection the wells were having their wellheads built. Patterson 580 moved off of the pad last week and these wells have not been stimulated. The pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
0350326460 2017-11-01 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 580 drilled a 17.5 inch diameter open hole to a depth of 634 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming casing ran 624 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Casing was centralized with 14 centralizers. Shoe track is 42 feet long. Schlumberger was onsite with 552 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 89 barrels of fresh water. 20 barrels of gel pumped ahead of cement. 30 barrels of cement returned to surface, and floats held with 0.5 barrels back.
1402420604 2017-10-19 Casing String Witnessed No Violations On the day of my inspection I witnessed express energy cement the 20 inch diameter conductor casing. A 30 inch diameter casing was set to 40 feet and the backside was ready-mixed. The pad is on strip ground and they had to stabilize the unconsolidated material to reach total depth. A 26 inch diameter hole was augered to 120 feet. Express cemented the 20 inch diameter casing with 288 sacks of class A cement. Cement weighed 15.6 with mud scale and has a yield of 1.18. Additives include 2% cacl and .5#/sk celloflake. 100% excess ran in open hole from 40 to 120. Cement displaced with 39 barrels of water. No cement returned to surface. Cement top will be measured tomorrow and remedial action will be discussed at that time.

2046969034 2018-05-07 Completion Testing No Violations On the day of my inspection I found a halliburton frac crew rigged up on the 201H and the 1H. During my inspection, I witnessed frac stage 63 of 75 on the 201H. 9.625 inch diameter annualr pressure of this well is 360 psi.
2049755443 2017-10-20 Casing String Witnessed No Violations On the day of my inspection I witnessed express energy top out the conductor annulus. Cement was pumped yesterday and did not return to surface. Cement was tagged at 27 feet from surface. 9-sack grout was poured down the annulus until it was full and the cellar floor was cemented also.
