0622560264 2015-10-06 Drill / Deepen / Reopen No Violations HWD Drilling Rig # 11 has just freed the stuck drill string. They are using air to unload the hole and to clean up the cuttings. Prior to getting stuck they had drilled to 385 ft. and had pumped two times the volume of the hole and did not get circulation before the drill string got stuck. I ask if they had encountered a mine and Matt Stevenson the company man said no. I ask about the lack of circulation and he said that they never lost returns on air but did not get circulation before getting stuck. He wants to try and get the drill string free and clean up the hole on air. They will trip the hammer out and go back in with a 17.5 inch diameter tricone drilling bit with no jets. When the hole has been cleaned up they will try and establish circulation. If they do, he will make sure there is no fluid being lost. After the well bore is static they will trip out to run casing. I advised that circulation was a must before running casing.
0755029642 2015-10-06 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 13-3/8 inch diameter casing string. HWD # 11 drilled a 17.5 inch diameter hole to 385 ft. and set 362 ft. of 54.5 lb. casing. Had to wash casing to bottom. After pumping 800 bbl. Of freshwater they could not get totally to bottom. During the wash down fluid was dropping in the hole and could not circulate to surface. I advised that the cement job could not be done until circulation was static. Next they mixed lost circulation material or LCM approximately 400 sacks of fiber and cedar chips. The well did not circulate to surface so more LCM along with bentonite gel was ordered. It arrived and the mix material ratio per bbl. of freshwater will be 8 sacks of gel, 15 sacks of cedar chips, 5 sacks of nutshell chips, 5 sacks of Mix 11 material, 2 sacks of Vin Seal. They are mixing up a pill that should weight 40 lbs. per bbl. Mix 50 bbl. of pill to sweep the well bore and establish circulation. The first pill did not bring fluid to surface and they lost 350 bbl. of fluid and the pill. They mixed another pill of the same material and also 50 bbl. Total with the same results. I left after the last loss of material and they are to call me when they establish circulation.
1070674957 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 wells. Current operations on the pad are installing frac stacks on the wells in preparation for well stimulation in April. Outslopes of pad are vegetated and stable. A crane and frac tanks are on location. Site meets requirements of ORC 1509.072A for preliminary restoration.
1315152685 2018-08-28 Production Wells No Violations On the day of my inspection I am viewing pad for complaint 5946. Royalty owner is concerned they are not going to get paid for production because lease is apparently transferring to Ascent resources. Pad contains 5 plunge lift wells with valves open. Production equipment includes 5 GPUs, a compressor, and three 400 barrel steel tanks in their own containment. Surface annular pressure is 0 and intermediate annular pressure is 0. Spoke with landowner after royalty owner after inspection and he believes the situation has been corrected. Complaint can be closed.
1444059681 2015-10-05 Drill / Deepen / Reopen No Violations I received a call from Horizontal Well Drilling Rig # 11 that they would be ready to cement casing around 4 A.M. I arrived to find that they are having problems and would not be running casing today. They did drill to 385 ft. on air. They were not having returns even on air. Then they became stuck in the hole. While trying to pull off bottom the drilling line popped off the drum and the crown sheaves. I ask if they encountered a mine void and they said that they have not. They have not tried to circulate at this time and I explained that this is what happened on the last well with the lost circulation and no returns. The permit requires them to set a mine casing string if they encounter a mine void or the influence of a mine void. Drilling without returns could seal off the void somewhat but would not protect the casing from acid mine water and would put the Surface Casing String in jeopardy if done that way. They did establish circulation on the last well but only after mixing large amounts of lost circulation material. I advised that due to the fact that they did not have returns on air meant that there is a void and should be dealt with as a mine void. They are to fix the drilling unit and then notify me after trying to establish circulation. I contacted Permitting Manager Steve Opritza to ask about the under ground mine overlay. Steve feels that there is an underground mine in that area. Steve advised me he had amended the permit to include a mine string as part of the permit.
1444515558 2015-10-10 Casing String Witnessed No Violations Upon arrival on location I found the rig crew has drilled a 17 1/2 in diameter wellbore to depth of 535 feet. They ran 520 feet of 13 3/8 inch casing weighing 54.5 lbs. into the wellbore and cemented back to surface with 550sacks of cement. Established circulation and circulated 29 bbls of good cement returns to surface. Landed the plug at 220 psi, and pressures to 820 psi. Held pressure for 5 minutes and released with 1/2 bbl of water back in the tanks. Floats held.
1445183577 2015-10-16 Casing String Witnessed No Violations I am on site this morning to witness the running and the cementing of the 9-5/8 inch diameter casing string. HWD# 11 drilled a 12-1/4inch diameter hole to 5000 ft. and set 4980 ft. of 40 lb. casing. Schlumberger Cementing Service cemented with 660 sacks of class H cement at 12 lb. and a yield of 1.97 lead cement and 490 sacks of class A cement at 15.6 lb. and a yield of 1.22. - Had to wash casing to bottom and then had to wait on water for the cement job. Circulated 42 bbl. Of good cement to surface.
1523479093 2016-01-25 Drill / Deepen / Reopen No Violations I arrived on location to check on the drilling status of Nabors Rig No.36. I met with Company Man John Mode. John informed me that Nabors was in the process of drilling the horizontal section of the well bore. The production casing was set to be run in 2 days. While on site I visually conducted an inspection of solids control where I found everything clean and in order. There were no leaks or spills outside of containment.
1525970773 2018-05-07 Completion Testing No Violations On the day of my inspection US Well Service was onsite rigged up to the 3h, 4h, and 5h wells. This well has already been fracked. Well has 0 psi on intermediate annular and 0 psi on the surface annular. There is no fracking ongoing at the time of the inspection.
1549321638 2015-10-13 Drill / Deepen / Reopen No Violations HWD Rig # 11 is drilling ahead on the 12-1/4 inch diameter section of the well. They will drill to 5000 ft. and set 9-5/8 inch diameter casing. While on site I checked the solids control area and found everything is clean and operating on containment. There has been no loss of material off containment. Because of the loss of 2000 bbl. Of water during the last casing installation I went to as many of the ponds, feeder streams and creeks in a circle around the drilling operation to see if I could observe any signs of those fluids. I did not find any thing which would indicate that they came from the drilling operation such as drilling soap or drilling muds or riled up fluids. During the fluid loss event they used large amounts of LCM or lost circulation material which would be easy to observe in a stream or pond.
1596851717 2015-10-08 Casing String Witnessed No Violations I arrived to witness the running and the cementing of the 18-5/8 inch diameter casing string. HWD # 11 had to pull out the 13-3/8 inch diameter casing and go back in the hole to drill out the hole so that they could set the 18-5/8 inch diameter mine string. They were never able to establish circulation and needed to set the mine string to isolate that zone. Matt Stevenson the Company Man for Hess Ohio called to advise me that they had reached total depth of the hole and would be running 215 ft. of casing as the mine string. I arrived to find that they were running casing and that they had two joints in the hole. They could not get the casing to screw together. After waiting most of the day and evening they still had 6 joints of casing to run so I was not able to witness the cement job.
