0469744735 2016-03-07 Drill / Deepen / Reopen No Violations On the day of my inspection while accompanied by inspector Mark Moreland, I witnessed Nabors Rig No. 36 was 95 percent completed in the process of rigging down. A steaming contractor was on site cleaning all drilling equipment and multiple Hess Employees were assisting in the rigging down process. While on site Mark and I did not witness any signs of spills or contamination.
0505966425 2018-08-28 Production Wells No Violations On the day of my inspection I am viewing pad for complaint 5946. Royalty owner is concerned they are not going to get paid for production because lease is apparently transferring to Ascent resources. Pad contains 5 plunge lift wells with valves open. Production equipment includes 5 GPUs, a compressor, and three 400 barrel steel tanks in their own containment. Surface annular pressure is 0 and intermediate annular pressure is 180. Spoke with landowner after royalty owner after inspection and he believes the situation has been corrected. Complaint can be closed.
0901483284 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 wells. Current operations on the pad are installing frac stacks on the wells in preparation for well stimulation in April. Outslopes of pad are vegetated and stable. A crane and frac tanks are on location. Site meets requirements of ORC 1509.072A for preliminary restoration.
1435755260 2015-06-30 Casing String Witnessed No Violations HAD Drilling RIG # 618 is drilling a 24 inch hole and setting 24 casing at 100 ft. with no cement because it is drilling using the concentric drilling method with no annular space to cement.
1447022884 2015-11-08 Casing String Witnessed No Violations Inspector Moreland and I are on site to witness a mine string cement job on HWD (Horizontal Well Drillers) Rig No. 11. HWD drilled a 22 inch diameter hole to 196 feet and set 172 feet of 18 5/8 inch diameter, 87.5 pound casing. There are two cement baskets on this job, one at 92 feet and one at 72 feet. There is a float collar at 152 feet. Schlumberger is performing the cement job, using 220 sacks of class A 15.6 pound, 1.2 yield cement. This volume should have been enough to bring cement returns to surface, however there were no returns of any kind witnessed at surface. Next HWD placed 1 1/4 inch diameter tubing in the annular space to 60 feet, then Schlumberger pumped 110 sacks of class A cement and topped off to surface. Inspector Moreland and I witnessed good returns to surface.
1447094480 2015-11-09 Drill / Deepen / Reopen No Violations I am on site today with Inspector Moreland to witness a surface casing cement job. HWD (Horizontal Well Drillers) Rig No.11 drilled a 17 1/2 inch diameter hole to a depth of 370 feet and set 352 feet of 13 3/8 inch diameter, 54.5 pound casing. Shlumberger was late arriving to the location, then took two hours to rig up. Prior to beginning the cement job, a quality control analysis was conducted on the cement. The casing job was then placed on hold, because the cement was thought to have failed the test. We were unable to witness the casing cement job because there was no time frame for the remedial action of the failed quality control test.
1634481007 2015-11-10 Drill / Deepen / Reopen No Violations Arrived on location to meet with company man Matt Stevenson to review the remedy to the cement problems in regards to the failed quality control test. The cement job report indicated that the intermediate casing string cement job was started around 11 pm and concluded approximately around 12:30 am. The report also indicated that 16 bbl. Of cement was returned to surface.
2031266159 2016-03-09 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by inspector Mark Moreland, I witnessed Nabors Rig No. 36 was completely rigged down. While on site I spoke to the company man Roy Chaney who informed me that the rig will be laid down in a yard in Pennsylvania and Hess Ohio has no plans to drill in Harrison County Ohio for the rest of the year. While on site I did not see any signs of spills or contamination.
2065031563 2018-05-07 Completion Testing No Violations On the day of my inspection US Well Service was onsite rigged up to the 3h, 4h, and 5h wells. This well is on the 29th out of 62 frack stages. Well has 295 psi on intermediate annular and 0 psi on the surface annular. There is no fracking ongoing at the time of the inspection.
