HWD Rig No.11 drilled a 17.5 inch diameter hole to a depth of 360 feet, and set 352 feet of 54.5 pound casing. Schlumberger cementing service, cemented with 360 sacks of class A 15.6 pound cement with a yield of 1.20 percent and circulated approximately 40 bbl of good cement to service
1080831968 2015-11-02 Drill / Deepen / Reopen No Violations Inspector Moreland and I arrived to pick up cement information from the company man Matt Stevenson. Matt informed us that HWD Rig No.11 was at a depth 5161 getting ready to drill out the cement shoe, to follow with a F.I.T(formation integrity test). While on site conducted a solids control check, to find all solids were contained and the location was clean.
1374382723 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 wells. Current operations on the pad are installing frac stacks on the wells in preparation for well stimulation in April. Outslopes of pad are vegetated and stable. A crane and frac tanks are on location. Site meets requirements of ORC 1509.072A for preliminary restoration.
1445535664 2015-10-22 Drill / Deepen / Reopen No Violations Witnessed HWD Rig No 11. drilling of a 17 .5 inch diameter hole to 195 feet. Then witness the trip out of hole and put a 22 inch reamer bit on, in order to install 18 5/8 inch diameter mine string casing which will occur later on in the night, will return to witness the casing cement job
1445595779 2015-10-23 Drill / Deepen / Reopen No Violations Inspector Moreland and I arrived on location to witness a mine string casing cement job. Upon arrival we met with the company man Jim Neal. Jim had advised us that they were encountering problems down hole with the mine. Jim did not advise us of the problems prior to our scheduled mine string time. HWD ( Horizontal Well Drillers) was encountering large volumes of water and a possible roof collapse of the mine, at the depth of 136 to 143 feet. At this time HWD drilling were retrieving drilled pieces of lumber material, thus providing factual evidence that indeed they had drilled into an underground mine. To assess the problem the company had planned to trip out the 22 inch diameter reamer bit and trip in with a 17inch diameter tri-cone bit in order better handle the material downhole. HWD drilling will be required to contact inspector Moreland when they have solved downhole issues and are ready to cement the mine string.
1445732091 2015-10-24 Casing String Witnessed No Violations Upon arrival on location I was accompanied by Inspector Joe Zollars. The rig crew was ready to set a mine string at a depth of 165 feet, with a cement basket set at 85 feet with one cement basket under the other one. In pumping cement of the bottom we encountered high pressure and pumped at a low rate of 1 bpm. Was unable to pump the projected number of sacks of cement, pumping 25 bbl. Of cement. Ran 1 inch tubing to a depth of 62 feet and grouted the top with returns to surface.
1445899887 2015-10-26 Drill / Deepen / Reopen No Violations Inspector Moreland and myself are on site this morning, because we were advised that the surface casing was to cemented around noon today. Hwd drilling encountered problems with drilling conditions and moved the surface cement job to later hours. Inspector Mooreland and I will return tonight to witness the surface cement job.
1446816278 2015-11-06 Drill / Deepen / Reopen No Violations HWD (Horizontal Well Drillers) Rig No.11 drilled to a depth of 7294, then set a plug using a wireline service truck and performed a pressure test. The pressure test will test the integrity of the formation and also the integrity of cement jobs performed on the well bore. Currently HWD is in the process of nippling down and moving to the Athens 4-H.
1683339247 2018-08-28 Production Wells No Violations On the day of my inspection I am viewing pad for complaint 5946. Royalty owner is concerned they are not going to get paid for production because lease is apparently transferring to Ascent resources. Pad contains 5 plunge lift wells with valves open. Production equipment includes 5 GPUs, a compressor, and three 400 barrel steel tanks in their own containment. Surface annular pressure is 0 and intermediate annular pressure is 0. Spoke with landowner after royalty owner after inspection and he believes the situation has been corrected. Complaint can be closed.
1692089054 2015-12-23 Drill / Deepen / Reopen No Violations On the day of my insepection I found that HWD ( Horizontal Well Drillings) Rig No.11, had completed their rig down down process and all drilling equiment was off site. Steel protection boxes were placed over top of every well bore that was drilled on this pad. There were no signs of poullation/contaimation on location. Hess Ohio plans to move Nabors Rig. 36 on location to finish the drilling process for all well bores on this pad.
1706382513 2016-02-04 Drill / Deepen / Reopen No Violations I arrived on location to check on the drilling status of Nabors Rig No.36. I met with Company Man Roy Cheaney. Roy informed me that Nabors was in the process of drilling the horizontal section of the well bore. The production casing was set to be run the next day. While on site I visually conducted an inspection of solids control where I found everything clean and in order. There were no leaks or spills outside of containment.
1938860046 2018-05-07 Completion Testing No Violations On the day of my inspection US Well Service was onsite rigged up to the 3h, 4h, and 5h wells. This well is on the 39th out of 61 frack stages. Well has 270 psi on intermediate annular and 0 psi on the surface annular. There is no fracking ongoing at the time of the inspection.
2075189938 2016-02-10 Drill / Deepen / Reopen No Violations I arrived on location to check on the drilling status of Nabors Rig No.36. I met with Company Man Roy Cheaney.
