1086726498 2015-07-20 Casing String Witnessed No Violations On location to witness the cementing of the surface casing. The rig ran 13-3/861# R-3 casing to 1305 O-Tex Pumping cemented it with 1350 sacks of Class H cement with 2% CaCl2 and .2% X-AiP (defoming agent) and 1/4 #/sack flake. 90 bbls of good cement was returned to the surface. The casing was tested to 500 over the landing pressure for 5 minutes and held. The pressure was released, and the floats held.
1134427418 2015-04-04 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Rat Hole Service drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb. casing. Universal Well cemented with 310 sacks of class A cement at 15.6 lb. and a yield of 1.18 - had good cement returns.
1181086682 2015-07-26 Casing String Witnessed No Violations On location to witness the 2 stage cementing of the 9-5/8 intermediate casing. The rig ran the casing to 7380. O-Tex Pumping cemented the first stage with 500 sacks of Low Dense H lead cement followed by 360 sacks of Premium H cement. The 2 stage cementing tool was opened and cement was circulated out of the annulus. The the 2nd stage was cemented with 1200 sacks of Premium H cement. 54 bbls of good cement was returned to the surface. See the casing ticket for the details.
1303711297 2017-04-12 Preliminary Restoration No Violations Onsite with Inspector Randy Akins. Purpose of this inspection is a status check. We performed a walk around of the drill pad and no found issues. Containment has ten 500 barrel tanks, seven are condensate and three are produced water. Production equipment including flares, compressors, heater treaters, and seperators on site. Well is producing with a christmas tree type wellhead with 150 psi on surface casing annular and 250 psi on intermediate annular.
1565861350 2015-07-27 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 284 is drilling a 8.75 inch diameter hole building the curve at 7720 ft. I also conducted an inspection in and around the chemical storage area and the solids treatment area. The chemical storage area was inside a secondary duck pond containment area with no leaks or spills outside. The solids area was clean with no leaks or spills.
