0146483449 2016-09-21 Production Wells No Violations I am on site today after receiving a call from an employee of SPSI, which is a spill cleanup company. The employee informed me that there had been a spill on 9/16/2016 and he was unsure who to call to make sure the spill was properly excessed. He informed me that the spill was reported to Ohio One Call the day it had happened and the spill occurred at the end of the lease road where it meets the county road, when a production water hauler flipped over. On the day of my inspection I witnessed that the spill had occurred on the side of the road and salt crystallization was evident in the area of the spill. After being unsure of the jurisdiction, I placed a call to Trish Cartmell. Trish informed me that the spill had not been properly reported and that she would take care of the release.
0504915255 2015-06-21 Casing String Witnessed No Violations Inspector Todd Miller was on site to witness the 1st stage of this two stage cement job. Patterson Drilling Rig # 284 set 7204 ft. of 9-5/8 inch diameter casing and cemented stage one with 510 sacks of class H cement at 12 lb. with a yield of 2.15 of lead cement and 360 sacks of class H cement at 15.3 lb. with a yield of 1.26 of tail cement. The D.V. Tool was opened and cement was circulated from 4000 ft. to surface. O-Tex continued circulation for 8 hours and then cemented with stage two. First 50 bbl. Of Barite was pumped at 12.5 lb. followed with 1212 sacks of class H cement at 15.3 lb. and a yield of 1.26 and circulated cement to surface. Job used total of 2082 sacks
1239930323 2016-12-21 Production Wells No Violations On the day of my inspection I witnessed Oak Hill is in the process of rigging down their flow back equipment. The production equipment has been installed for the well bores and the wells appear to be in producing order. Due to the high amount on employees on site I did not conduct an inspection of what the production equipment consist of. I will be back on site at a later date to gather more information about the well location. While on site I did not see an signs or indications of pollution or contamination.
1428148765 2015-04-04 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb. casing. Universal Well cemented with 310 sacks of class A cement at 15.6 lb. and a yield of 1.18 - had good cement returns to surface.

1434973081 2015-06-20 Casing String Witnessed No Violations On location for the first stage of a 2 stage cement job. The rig ran 9-5/8: 40 # casing to 7207 (hole total depth 7211). Float shoe on bottom, float collar 2 joints up (shoe track = 91) The cement stage tool was at 4011. O-Tex Services pumped 50 bbls of Barite Spacer at 11.5 ppg. 195 bbls of lead low dense cement was then pumped at 12 ppg followed by the tail cement at 15.3 ppg. The cement was diplaced with 539 bbls of Oil based mud. The plug landed at ~620 psig and was bumped to 1120 psig for 15 minutes. No drop off. Upon release, the floats held. The stage tool opening bomb was dropped and after 30 minutes the tool was opened with 850 psig and circulation established. The rig mud pump circulated the spacer and cement out of the hole. They continued circulation until the 2nd stage was to start. (0345 hours)

1492019561 2017-04-12 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Randy Akins I witnessed metal containment has ten 500 barrel tanks, seven are condensate and three are produced water. Production equipment including flares, compressors, heater treaters, and seperators on site. Well is equipped with christmas tree type welhead with open valves. Valves show 300 psi on surface casing annular and 750 psi pressure on intermediate annular.
1858065154 2016-06-13 Casing String Witnessed No Violations I am on site this morning to witness the running and the running of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 284 drilled a 17.5 inch diameter hole to 1970 ft. and set 1970 ft. of 61 lb. casing. O-Tex Cementing Service cemented with 270 sacks of lead cement at 15.6 and a yield of 1.19 and 1210 sacks of class H cement at 15.6 lb. and a yield of 1.20 and had 40 bbl. Of good cement returns.
2114186257 2015-06-10 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 18-5/8 inch diameter casing string. Patterson Drilling Rig # 284 drilled a 26 inch diameter hole to 430 ft. and set 433 ft. of 87.5 lb. casing. O-Tex Cementing Service cemented with 980 sacks of class H cement at 15.6 lb. and a yield of 1.20 and had good cement returns to surface.
